Ee was stepping onto a piece of granite grout near a creek when the piece of grout rolled over causing him to fall thus hitting his left arm/elbow area on another piece of granite grout causing a compound fracture to his left arm/elbow. He had two plates and 14 screws implanted in his arm during surgery.

AFTER SHUTTING MACHINE OFF, EE WAS EXITING THE GALION DRILL WHEN HE SLIPPED AND FELL THUS HITTING HIS LEFT ELBOW, LEG AND ANKLE ON THE OUTSIDE OF THE MACHINE. HE RECEIVED A DEEP BRUISE TO HIS LEFT ELBOW AND WAS PLACED ON LIGHT DUTY.

A LOAF SHOT HAD BEEN MADE THE LAST THING ON 5-5-97. THE LOAF OPENED UP APPROX. 30" FROM THE WALL. THE NEXT MORNING, EE AND SEVERAL COWORKERS WERE WALKING TO THEIR WORK STATIONS WHEN EE STEPPED OFF IN THE 30" GAP AND FELL APPROX. 16' TO THE QUARRY FLOOR. EE SUFFERED A FRACTURED LEFT ANKLE. THE LIGHTS IN THE QUARRY HAD JUST BEEN TURNED ON AND WEREN'T FULL ILLUMINATED AT THE TIME.
