Welder was air arcing a slope board mount off and was struck by push arm after it had fallen from stands. This resulted in an abrasion to EE's right arm as well as fracturing EE's right foot pinky toe and adjacent toe.

Employee was using a hose reel to reel up a 2" water hose at the wash bay. Employees right thumb was caught between the manual reel handle and reel locking mechanism resulting in a laceration to the top of this thumb. This injury required sutures

Employee was exiting the crew bus at the ready line prior to start of shift. EE lost balance and hurt right ankle when stepped out on to the ground.

Employee was using an o-ring pick, it slipped and stuck EE in the hand. EE looked and the wound, became light headed and fainted to the ground. EE was taken to the hospital, no head trauma found, given tetanus shot and antibiotics for precaution. EE was released same day to return to work without restrictions.

Employee was riding in a CAT 745 haul truck while training. While going through a rough area the employee reached forward to grab the hand hold. EE's hand contacted the hand hold with some force and caused discomfort and swelling in EE's left wrist. Employee was taken to the Work Med Clinic and was diagnosed with a non displaced mid scaphoid fracture in the left wrist

Haul truck operator was crossing the dump area and drove through a soft spot the seat suspension bottomed out jarring the operator causing discomfort in the lower back, employee returned to work on own accord. On 6/10/19 the discomfort had increased over night and the employee was taken to Portneuf Workmed for evaluation and received chiropractic treatment.

Experiencing numbness in left hand for several months causing pain and discomfort. In the past few weeks, the pain has escalated and has been keeping ee awake at night. The pain is potentially working into the wrist and forearm region. Reported on 5/25/2017, diagnosed as Cubital Tunnel Syndrome

A mechanic was removing an equalizer pin on a D9 dozer. A small hand punch was used to finish tapping out the pin. When the pin cleared, the right track dropped and equalizer bar on the left lifted up pinching the mechanic's left middle finger between the punch and the side frame bore. The employee's finger received a broken bone and laceration resulting in 9 stitches.

A 992 operator strained back as EE was sitting down in the operator's seat. EE was getting into the seat to adjust it and start the machine when EE felt discomfort in lower back. An onsite EMT was called to examine the employee. Upon further examination, a doctor would not release EE to full duty due to a back strain.

Employee was feeding ore into the grizzly and when he was backing up to make another push he backed over a rock and struck his right elbow on a metal plate where a padded armrest should be. Operator was taken for x-rays which were negative. 8/2/15 fluid build-up from bursitis was drained by physician along with prescription medication

Employee notified he was diagnosed with carpal tunnel in both extremities. He worked through it until he could not deal with discomfort. On 1/7/2016 he had surgery to repair problem.

Employee was performing his walk around when his left foot slid forward; as he was falling he was able to stabilize himself with his right leg preventing him from hitting the ground. After the event happened he felt some discomfort in lower back. Follow up with doctor on 1/22 and prescription medication was issued to help with discomfort. MRI sceduled

Fine metal slivers and disc debris fell out of employee hard hat and into safety glasses. While returning home, he rubbed his left eye and felt a sharp pain. When the employee returned to work the next shift, the supervisor noticed the employee's red irritated eye. Onsite EMT examined and employee was taken to the hospital and a metal sliver was extracted from eye.

Employee pulled a service truck into the PM bay for servicing. Employee drained engine oil and was starting to change oil filters. Employee was lying on their back under the truck reaching up to change an oil filter when a bee fell down from the frame rail into the coverall sleeve and stung the employee on the right forearm.

A 6' step ladder was used to access the right side door of a trailer. As the employee repositioned himself, he lost his balance and his feet slipped off the edge of the door jamb causing him to fall approximately 5' to the ground. As he fell, his back contacted the outer edge of the door jamb causing an abrasion/impact wound resulting in 3 broken ribs.

Employee was cutting a twine string which was holding a straw wattle together. He was walking while doing this and slipped on a mud clump while cutting. This action, coupled with the upswing of the pocket knife he was using caused the knife to lacerate his left lower arm. It took 7 sutures to close the wound.

Employee was changing cutting edges on a D10 dozer with the assistance of two mechanics. The bar that the injured employee was using slipped and the cutting edge came down crushing the employees right ring finger in between the edge and the dozer blade, amputating the tip of the finger off.

Employee was installing one section of a rolled dozer liner, setting the liner into place and as he moved the plate towards the face of the dozer, the lifting eye that was welded on the top edge of the liner broke free hitting the dozer face, and sliding onto the welder's left foot. X-ray taken on 7/11/12 indicated a non-displaced fracture of a metatarsal bone in left foot.

Technician was trying to raise a grader with a double action hydraulic lift. One hose was disconnected from the lift. When the lift was pressurized, the return coupler failed and ejected a ball bearing which struck the technician in the left shoulder.

Employee was holding a wrench on a nut as the bolt was being driven in with an impact wrench. As the bolt went through the nut, a portion of the thread dislodged and became embedded in the employee's left index finger. A PA-C had to extract the foreign object.
