EMPLOYEE WAS OPERATING A BATTERY POWERED, TRACK MOUNTED MANTRIP. UPON ENTERING THE MINE, THE WHEELS OF THE MANTRIP APPARENTLY LOCKED UP AND THE VEHICLE BECAME OUT OF CONTROL. THE MANTRIP STRUC K A METAL DOOR AND THE DOOR STRUCK THE EMPLOYEE.

THE INJURED EMPLOYEE WAS OBSERVING THE INSTALLATION OF AN ADJUSTING JACK TO THE SIDE OF A BELT DRIVE BOOM. THE BOOM SLIPPED ON CRIB BLOCKS AND FELL APPROX. 8". THE EMPLOYEE CAUGHT HIS LEFT HAND BETWEEN THE BOOM AND A CRIB BLOCK (FRACTURED 3RD & 4TH METACARPALS).

Employee was removing a drill wrench from the drill pot when a small piece of draw rock fell and struck his left ring finger near the tip. This caused a laceration that required sutures.

Employee was operating a rail ride when a piece of draw rock fell and bounced into the operator's compartment striking him on the left side of his head. 5 Staples were required to suture the wound.

The employee was holding a piece of conduit with a pair of channel locks to split the conduit with a razor knife. The channel locks slipped causing him to lacerate his left hand between the thumb and index finger. This required 6 sutures.

Employee was changing out a hydraulic motor on a scoop and got his left hand caught between the motor and scoop frame when he dropped the motor. His left little finger sustained a fracture. He did not miss any work and is performing his normal duties.

EE WAS INSTALLING BOLTS ON HELPER SIDE OF A DD015 FLETCHER BOLTER IN THE #2 FACE. AS HE WAS PUSHING UP THE BOLT, A LOOSE PIECE OF DRAW BROKE AROUND THE BOLT (APPROX SIZE 1' X 6" X 3"), FELL AND STRUCK EE ON HIS HAND. BROKE EE'S RING FINGER.
