Enterprise Mining Company LLC · Over-exertion in lifting objects

Employee was installing gate on mine road and while lifting on gate became unbalanced. Employee originally starting missing on 4/21/2007 for personal illness. Employee notifed employer on 6/25/2007 he was going to seek medical treatment for back strain which occured on 4/20/2007.
