HE WAS STANDING NEAR THE CONVEYOR BELT INSPECTING THE FEEDER & CONVEYORS. A ROCK FELL OFF & HIT HIM ON THE HEAD & SHOULDER. HE HAS A CUT ON HIS HEAD WHICH REQUIRED STITCHES. BRUISED SHOULDER.

HE AND ANOTHER EMPLOYEE WERE TRYING TO POUND A PIN OUT OF THE LOADER. HE WAS HOLDING A BOLT. WHEN THE PIN CAME OUT, THE LOADER BUCKET FELL, THE BOLT SMASHED HIS HAND AGAINST THE LOADER.
