When exiting a Volvo 220 loader employee caught boot on a step and fell to the ground. Accident resulted in small muscle tear in left shoulder. Employee failed to maintain 3 points of contact. The step was inspected and found to be in good condition.

Operator was charging a battery in a loader and it exploded. Minor burns on face small cut on THE chin. Operator was wearing safety glasses so other injuries were avoided.

HE WAS OPERATING A LOADER FEEDING GRAVEL TO THE CRUSHER. HE HAD JUST DUMPED HIS LOAD AND WAS BACKING UP. ANOTHER LOADER WAS COMING IN. THE TWO LOADERS COLLIDED. HE HAD A TINGLING SENSATION IN HIS LEFT LEG BUT THOUGHT IT WOULD GET BETTER SO HE DID NOT GO TO THE DR. HE CALLED THE OFFICE ON 1-28-03 TO TELL HIS LEG PAIN WAS WORSE AND HE HAD GONE TO THE DR ON 1-27-03.

HE WAS KNEELING DOWN TO WELD ON CRUSHER FEEDER. WHEN HE STOOD UP HIS KNEE LOCKED UP AND WOULDN'TSTRAIGHTEN OUT.
SHE WAS GOING START THE GEN SET SO SHE COULD PUT FUEL INTO THE LOADER AT THE END OF HER SHIFT. WHEN SHE MOVED A SHAFT THAT WAS LEANING ON THE TRAILER SHE SMASHED HER FINGER BETWEEN THE SHAFT A DN THE TRAILER.

HE WAS PICKING CRUSHER SCREENS OFF THE SKID STEER LOADER & PUTTING THEM INTO THE CONTROL VAN TO MOVE TO THE NEXT LOACATION. HE TWISTED HIS BACK AS HE WAS LIFTING & MOVING THE SCREEN. HE DISLOC ATED A DISC WHICH IS PINCHING A NERVE.

HE WAS TRYING TO PUT THE FLASHING ON THE CONVEYOR BELT AT THE CRUSHER. HE WAS PUSHING ON THE FLASHING WITH A CROWBAR. THE BAR CAUGHT ON THE CLIP ON THE BELT AND PUSHED HIM BACK AGAINST THE CRU SHER. HE WAS BRUISED ON HIS CHEST, NECK AND LEFT SHOULDER.
