Riverview Sand & Gravel, L.L.C. · Caught in, under or between (Not Elsewhere Classified)

This was an accident that occurred with prior employee under prior management. I have spoken to the former employee to gather this information. On November 15, 2013 the employee was removing a cover from a conveyor and broke his pinky finger on his left hand. We no longer own the conveyor that was involved in this incident.

Riverview Sand & Gravel, L.L.C. · Fall to the walkway or working surface

EE was walking down a side of hill next to conveyor. EE slipped in mud and reached out to grab conveyor to prevent fall. EE fell to ground anyway and dislocated left shoulder.
