EMPLOYEE STATED THAT HE WAS OPERATING A SHUTTLE CAR WHEN HE CAME OFF THE FEEDCER AND HIST A HOLE IN THE ROADWAY. THIS THREW HIM UP AGAINST THE CANOPY OF THE SHUTTLE CAR JAMMING HIS NECK.
EMPLOYEE STATED THAT HE WAS DRILLING HOLE IN THE ROOF AFTER HE DRILLED THE STEEL UP AS HIGH AS IT WOULD REACH HE PUT A WRENCH ON THE STEEL TO FINISH DRILLING THE HOLE THE WRENCH CAME LOOSE EEL STARTED SWINGING AROUND AND JERKED EMPLOYEES WRIST CAUSING INJURY
