On 11/12/13 EE was unplugging sections of the material pipeline from dredge. On a trip from the dredge to the bank, EE exited Jon Boat, his feet slipped on muddy bank. EE put left hand down to brace himself, due to force his arm slipped. EE saw Ortho on 3/13/14 diagnosed as shoulder tendonitis/labrum sprain. Ordered physical therapy 4 weeks and EE was released w/out restriction.

EE went down to the booster pump on the dredge to check for a stuck rock. While attempting to unbolt the inspection plate off the pump, the locking mechanism on the 3/4" drive ratchet slipped causing the EE to hyperextend his left shoulder resulting in a strain. EE did not see a doctor until 9/18 adn MRI taken on 9/22.

EE WAS HAMMERING THE OCUPLING ON DREDGE EE WAS HIT IN THE LEFT SMALL FINGER WITH A SMALL FRAGMENT OF METAL FROM THE HAMMER. HE WAS WEARIN GLOVES AT THE TIME OF INCIDENT.
EE AND TWO OTHE REMPLOYEES WEE REPLACING SUCTION PIPE ON DREDGE, WHENN THE PIPE ROLLED AND CAUGH THIS THUMB BETWEEN THE PIPE AND HOIST CHAIN. PIPE WAS PROPERLY AND ADEQUATELY SECURED.

INJURED EMP AND WITNESS WERE REMOVING A BEARING FROM THE ROCK SCREEN. VICTIM WAS USING A SLEDGE HAMMER TO HIT THE ROD WITH. HE MISSED THE ROD AND HIT HIS HAND.
