THE EE WAS INSTALLING LINERS IN A CRUSHER WHEN HE FELT A POP IN HIS NECK HE WAS EXAMINED BY A DR. WHO RECOMMENDED RESTRICTED DUTY FOR A FEW DAYS.
THE EE WAS REPACKING A ROTOR VALVE, WHEN HE FELT PAIN IN HIS BACK. AT FIRST IT WAS NOT VERY BAD. HE REPORTED THE PROBLEM BUT DID NOT SEE A DR. UNTIL 1930 ON 11-30. THE DR. TREATED HGIUM AND HA S HIM OFF WORK.

AFTER THE ACCIDENT INJURED TOLD (NAME)C0-WORKER,HE WAS TRYING TO FREE SWITCH.APPARENTLY HE WAS IN SUCH A HURRY HE DECIDED TO FOREGO GOING AROUND THE MACHINE AND TRIED TO GO UNDER FORKS TO FREE THE DOWNWARD LIMIT SWITCH.
EMPLOYEE WAS USING AN AIR OPERATED NAIL GUN (DRIVER) TO REPLACE BROKEN BOARDS ON WOODEN PALLETS. HE STOOD THE PALLET IN A LEANING POSITION AND WAS HOLDING THE BOARD WITH HIS LEFT HAND AND OPE RATING THE GUN WITH HIS RIGHT HAND. APPARENTLY HE MISSED THE BOARD AND CAUSED THE GUN TO DISCHARGE INTO HIS LEFT HAND. NORMAL PROCEDURE IS TO LAY THE PALLET DOWN FLAT AND LAY THE REPLACEMENT
