EMPLOYEE WAS SETTING UP 55 GALLON DRUMS OIL. WORKED REMAINDER OF SHIFT. TAKEN TO EMERGENCY ROOM LATER THAT EVENING. WAS TOLD TO SEE FAMILY DOCTOR NEXT MORNING WHERE WAS TOLD HAS HERNIA AND WAS SCHEDULED FOR SURGERY 3-4-99.
EMPLOYEE WAS RUNNING ROCK TRUCK AND WAS JARRED. HE DEVELOPED PAIN IN LEFT SHOULDER AND EXISTED ROCK TRUCK TO TRY TO ELEVIATE. PULLING HIMSELF INTO THE ROCK TRUCK CAUSED MORE PAIN-LOADED AGAIN. PAIN GREW MORE INTENSE. SHIFT WAS OVER AT NOON. HAD ANOTHER EMPLOYEE DRIVE EMPLOYEE HOME.
TEARING DOZER APART TO PUT NEW BRAKES ON, SLIPPED ON WET TRACK AND FELL APPROX. 3-6' TO GROUND AND DOOR ON WHICH BOLTS WERE BEING LOOSENED FELL ON TOP OF HIM. FRACTURED PELVIS IN 2 PLACES, FRA CTURED LEFT ELBOW, FRACTURED RIGHT WRIST, FACIAL CUTS AND SCRATCHES.
