While assisting the mechanics with a dozer engine the cross member slipped and fell on right hand breaking the ring finger.

Employee was operating a 992G loader ran into a Komatsu dozer being operated by another employee. He was jolted and instructed to be seen by a doctor. Following the appointment, he returned to work that afternoon.

Battery of crusher caught on fire; employee lifted off cover of battery to aleviate fire when battery exploded causing battery acid to burn eyes and face; pieces of battery cut employee's face.

EMPLOYEE WAS SETTING UP 55 GALLON DRUMS OIL. WORKED REMAINDER OF SHIFT. TAKEN TO EMERGENCY ROOM LATER THAT EVENING. WAS TOLD TO SEE FAMILY DOCTOR NEXT MORNING WHERE WAS TOLD HAS HERNIA AND WAS SCHEDULED FOR SURGERY 3-4-99.

EMPLOYEE WAS RUNNING ROCK TRUCK AND WAS JARRED. HE DEVELOPED PAIN IN LEFT SHOULDER AND EXISTED ROCK TRUCK TO TRY TO ELEVIATE. PULLING HIMSELF INTO THE ROCK TRUCK CAUSED MORE PAIN-LOADED AGAIN. PAIN GREW MORE INTENSE. SHIFT WAS OVER AT NOON. HAD ANOTHER EMPLOYEE DRIVE EMPLOYEE HOME.

AS INJURED WAS ATTEMPTING TO CARRY THE LEA KING TANK TO A CLEAR AREA, ESCAPING FUMES WERE IGNITED BY A HEATER INSIDE THE STORAGE BUS CAUSING A FLASH FIRE. EXTENT OF BURNS TO FACE & HANDS NOT KNOW AT THIS TIME.
