THE EE WAS DISEMBARKING HIS LOADER WHEN HE FELL ONTO HIS RIGHT LEG. THE EE COMPLAINED OF A STRAIN TO HIS HAMSTRING AND LOWER BACK.

WHILE RUNNING HIS DRILLING RIG THE EE NOTICED HIS DRILL SPEED WAS TOO SLOW. HE ATTEMPTED TO REPAIR THE DRILL BUT DID NOT SHUT OFF THE AIR. HIS GLOVE GOT CAUGHT ON THE DRILL ROD AS IT SPEED UP. THIS CAUSED HIM TO TWIST HIS HAND AROUND THE DRILL ROD PULLING LIGAMENTS AND CUTTING HIS LEFT HAND SUPPORT FINGER.

THE LOADER OPERATOR WAS MOVING A PIECE OF STONE IN FRONT OF THE EMPLOYEES DRILLING RIG. THIS DISLODGED A PIECE OF STONE ON WHICH ONE OF THE LEGS WAS SITTING CAUSING THE DRILLING RIG TO FALL OV ER. WHILE FALLING OVER THE DUST COLLECTOR CAN HIT THE EMPLOYEE AROUND THE RIGHT SCAPULA.

EMP WAS KNOCKING A BIT ONTO A DRILL ROD. RAISED THE ROD TO HIT IT ON THE GROUND. WHEN HE KNOCKEDTHE ROD ON THE GROUND HE PINNED HIS FINGER BETWEEN THE ROD AND THE GROUND. THE EMP HAD TWO ABRAS IONS ON RIGHT HAND MIDDLE FINGER. HE REQUIRED NINE STITCHES.

EMPLOYEE WAS OPERATING A HAND HELD JACKHAMMER FOR APPROXIMATELY 4 TO 5 HRS THE DAY OF INJURY. AT 1:00 HE TOLD THE SUPERVISOR HIS WRIST WAS VERY SORE, BUT HE SAID HE COULD CONTINUE. ON 4-3-96 H E SAID HIS WRIST WAS STILL SORE. THE SUPERVISOR ASSIGNED HIM NOT TO DRILL BUT PRESSURE WASH EQUIPMENT.
