MAN WAS DRIVING 922B LOADER DOWN A ROAD WITH A STEEP GRADE. FOR UNKNOWN REASON, LOST CONTROL AND THE LOADER TURNED OVER AND THE RAPS SAFETY CAB FAILED AND COLLAPSED CAUSING FATALITY.

EE climbed up on to an excavator track and grabbed on to the door handle. The door was not latched, it swung open. As a result he fell to the ground landing on his right arm and shoulder, resulting in a break to his arm.
