He was walking across stone & slipped on mud, falling. Hit his right arm & shoulder. Went to Dr who said sprain, no recheck unless problems. He wanted to go back to Dr on 10/2/08. Had a note to return 10/9/08 for recheck. Went back to Dr 10/8/08 with lumbar strain & high restrictions with no walking. Out as of 10/9/08.
