Had a note to return 10/9/08 for recheck. Went back to Dr 10/8/08 with lumbar strain & high restrictions with no walking. Out as of 10/9/08.

EMPLOYEE WAS DRILLING A LIFT HOLE IN GRANITE BLOCK WHEN PART OF IT SHEARED OFF CAUSING EMPLOYEE & ROCK TO FALL APPROX. 10'. EE HAD HIS SAFETY BELT ON WHICH KEPT HIM FROM HITTING THE BOTTOM.
Employee bent over to pick up pump and his knee popped. EE did not tell anyone until 9-9-04 at 8:00 am. I took him to the doctor at 1:30 pm and ee is on light duty.
HE WAS PUTTING IN A SNAKE HOLE IN GRANITE TO HOOK CHAIN THROUGH. THE BIT WENT THROUGH OUT THE END OF GRANITE(END OF GRANITE BROKE OFF) CAUSING HIM TO FALL. HIT HIS ARM ON GRANITE.

WHEN FE NCE GAVE, HIS THUMB WAS ON FENCE POST AND FINGERS STRETCHED AS THUMB WAS HOOKED ON POLE, STRETCHING AREA BETWEEN THUMB AND POINT FINGER CAUSING AREA TO TEAR.
