THE VICTIM WAS PRONOUNCED DEAD AT THE SCENE. THE BODY WAS TRANSPORTED TO THE BREATHITT COUNTY CORONER & THEN TO FRANKFORT FOR AUTOPSY.

He was using air gun to tighten a bolt on miner, when air hose blew off and struck him above left eye
Pulling nose cone off D9T tractor, lift jack came out of ball on blade, struck right knee and pinned right leg against the back of the blade - resulting in a knee contusion.
Got off of loader to walk around front of loader bucket, tripped over bucket tooth falling flat and landing on wrist

Employee was assembling highwall miner-using sledgehammer & slate bar. Was prying with slate bar to align bolt holes on structure of 2nd deck, bar slipped & he fell on deck onto his behind.
after coming out of office holding pre-shift books, slipped and fell on back landing on steps of office porch
Returning to office, heavy fog resulted in bad visibility, truck left the roadway & struck a rock.

Took break from loading holes to go and relieve himself. Started to go over the hill from bench where work was being done, slipped on rock & went down over hill landing on a rock then rolling on down to catch bench.
Employee backed dozer into rock truck. Employee was pushing fill for rock trucks. Employee said neck and shoulder was hurting.
EMPLOYEE WAS CLEANING OFF BACK WINDOW OF ROCK TRUCK. EE FELL 8-10' OFF OF ROCK TRUCK & HURT HIS BACK. EE COULD NOT FEEL, HIS LEGS WERE NUMB.

Employee was packing lift cylinder on d-11 dozer. EE's finger was pinched between cylinder and blade.
Cut bearing from pin bore and was moving out of the way. The bearing had excessive amounts of grease causing it to slip out of mechanic's hand striking him in the left lower leg.
Employee was being loaded by 992G loader boom and it struck the side of the truck bed. EE was complaining of a sore neck.

EE WAS COMING DOWN LADDER ON THE ROCK TRUCK & HE SLIPPED OFF HURTING HIS LEFT SHOULDER & ARM.
