The employee and another employee were carrying a new screen up to the building. When they got to the top of the catwalk and stopped for a break employee noticed an uncomfortable feeling in employee's abdomen.

EE was attempting to get a piece of steel from the steel rack. EE had moved 2 pcs of channel to get to the pc of steel EE wanted & when EE moved the 2nd pc of channel the 1st pc of channel came back on finger in turn lacerating ring finger on left hand which required 7 stitches.

EE tripped on piece of angle iron sticking out from a pallet of parts by the old scale house after he returned a shove. He fell face down catching himself on his hands and rolled on the ground injuring his right arm. Wrist hurt a little and he shook it off. After about an hour he had pain and could not pick anything up. Took him for X-rays, small compound fracture around elbow.

Employee had greased drill rod threads, then reached in to remove dirt from table as Driller was moving drill rod carousel to drill rod catching employees right hand and forearm between drill rod and drill carousel causing bruising of forearm and laceration of right hand.

Injured employee was operating a quarry haul truck. For a reason not yet known the truck engine stalled on an incline. The truck rolled backwards onto a roadside berm and overturned on the passenger side. As a result the injured worker suffered a dislocated shoulder.

Employee was shoveling waste material under a conveyor when he felt pain in his right hip as he worked. chiropractic treatment was sought at first by didn't help. lost time began after treating with a physician who took EE out of work on October 27.

EE was lifting an empty oxygen bottle off of the welder truck when he felt pain in his right shoulder. Initially this was reported as first aid however after testing was performed it was determined that surgery was required. EE surgery is scheduled for 12/15/2010.

EE WAS SHOVELING MUD OUT OF CORNER OF TRUCK BED (R25 EUCLID). HE SLIPPED ON THE WET TRUCK BED AND FELL. HE DISLOCATED HIS RIGHT SHOULDER.

EE WAS IN THE BUILDING WHERE THE 42" CRUSHER IS LOCATED. HE WAS WALKING AROUND THE END OF THE MOTOR AND HIS FOOT SLIPPED AND HE FELL OUT THE DOOR THAT WAS PARTIALLY OPEN AT THE TIME OF THE ACC IDENT. HE FELL APPROX 15 FT.

YEMPLOYEE WAS WORKING IN GARAGE AREA TAKING BELL HOUSING OFF OF ENGINE. THE BELL HOUSING HIT THE FLOOR AND HIS HAND WAS UNDERNEATH. HE RECEIVED A LACERATION OF TH ERIGHT INDEX FINGER.

EMPLOYEE WAS DOING A WASH TEST IN THE LAB AND WAS BENT OVER THE SINK. HE HURT HIS LOWER BACK.

WHILE EMPLOYEE WAS ON HIS BACK UNDER THE KOMATSU SHOVEL INSERTING THE TRACK BOLTS DOWNTHRU THE FRAME THE EMPLOYEE THOUGHT MAYBE SOME DUST OR SOMETHING GOT INTO HIS EYE IT DID NOT BOTHER HIM UN TIL HE WAS ON HIS WAY HOME AT 3:30PM ON 02-02-94HE SAID HIS EYE WAS WORSE SO HE MADE AN APPOINTMENT WITH THE DOC THE DOC REMOVED WHAT LOOKED LIKE EMERY DUST OR SOMETHING OF THAT NATURE HE SAI

EMPL HOOKING UP WATER PIPE TO WATER PUMP.TRIED TO MOVE PIECE OF WATER PIPE AND IT SLIPPED FROM HAND FELLON LEFT FOOT.TOP OF LEFT FOOT BECAME BRUISED AND SWOLLEN.

USING SLEDGE HAMMER, POUNDING PIN, HAND GOT HIT ON THE CHAIN BY THE HAND LE OF HAMMER

EMPLOYEE WAS REMOVING STONE FROM THE PRIMARY CRUSHER WHEN ONE OF THE CURTAIN CHAINS FELL AND HIT HIM IN THE FACE, CHIPPING HIS FRONT TOOTH.

EMPLOYEE WAS WORKING ON A SHOVEL, USING A PRY BAR TO PRY THE SWING ADJUSTERS IN ORDER TO ADJUST THE SWING TRAVEL. THE BAR SLIPPED AND THE EMPLOYEE PINCHED HIS RIGHT HAND BETWEEN THE BAR AND T HE GUARD.
