EE was standing on an uneven dirt mound and upon stepping off of it, EE stepped on a pipe, slipped and shifted all EE's weight onto EE's left leg causing EE pain in EE's knee thereafter. EE was limping following the incident, rested EE's leg for a while before deciding to leave.

THE SKIDSTEER NEED A NEW RADIATOR WE WERE PUTTING HYDRAULIC COOLER ON TOP OF THE RADIATOR TO BE BOLTED TOGETHER WHEN THE HYDRAULIC COOLER SLID OFF THE FORKS OF THE SKID STEER AND HIT ME ON TOP OF MY HEAD, THEN SLID OFF MY HEAD AND CAME TO REST ON MY KNEE PINNING ME. OTHER EE PICKED IT OFF ME. I WAS DIZZY ABOUT A MIN. THE NECK MUSCLES WERE SORE IN BACK.

EMPLLOYEE WAS GOING TO CLEAN OUT MATERIAL AROUND CONVEYORS. HE REMOVED GUARDS BUT DID NTO SHUT DOWN CONVEYOR. THOUGHT HE WOULD UST BRUSH MATERIAL AWAY FROM ROLLER. ROLLER GRABBED HIS GLVOE AND PULLE DIN HIS ARM. WENT TO HOPSITAL, DIAGNOSIS, BRUISED LEFT ARM.
