A fully trained miner was checking oil on a D-11 Dozer when EE slipped on the track of the dozer and landed on their shoulder. The injury was not the result of any compliance issues nor were any protective guards involved.

Truck Driver was dumping a trailer of bypro, Driver dumped on UNLEVEL ground causing the load to shift and turn the truck & trailer on its side. Driver was is in the cab of the semi at the time and was not wearing EE's seat belt. EE then fell into the gear shift causing injury to EE's rib and chest area.
