A fully trained miner was checking oil on a D-11 Dozer when EE slipped on the track of the dozer and landed on their shoulder. The injury was not the result of any compliance issues nor were any protective guards involved.
Truck Driver was dumping a trailer of bypro, Driver dumped on UNLEVEL ground causing the load to shift and turn the truck & trailer on its side. Driver was is in the cab of the semi at the time and was not wearing EE's seat belt. EE then fell into the gear shift causing injury to EE's rib and chest area.

While installing a ripper tooth pin on a dozer a 1 centimeter metal fragment from a punch cut the operator on the right side of the head. The fragment was removed requiring one suture.

WHILE USING A 3/4 AIR WRENCH ON A BOLT TO REMOVE A COLLETT FROM 988-7 LOADER BUCKET WHEN THE COLLETT SPUN AND HIS FINGER ON HIS RIGHT HAND WAS CAUGHT BETWEEN THE BOLTS AND CUT HIS FINGER.
WHILE HOOKING A 3" WATER HOSE TOGETHER, ONE OF THE HOSES HAD SOME HOLES IN IT SO HE TOOK OUT A UTILITY KNIFE TO CUT OFF THE BAD END. THE KNIFE SLIPPED FROM THE HOSE & STABBED LOWER LEFT FORE A RM.
