Injured was shoveling material out of a hopper when EE felt pain in their wrist.

Injured reports that EE was dragging a 4" corrugated vacuum hose when EE felt pain in EE's back. Injured did not report injury until 7/10/2024 when EE requested medical attention. Injured was given work restrictions on 7/10 which made the injury reportable.

Injured was attempting to close the rear engine door on skid steer when they smashed a finger in the door, causing a fracture to the tip of the finger.

Injured was cutting holes in a rubber cover with a utility knife when the knife slipped and cut a finger.

Injured was bitten by an insect which caused their eye to swell shut.

Injured was assisting a barge movement when a rope became pinched in a barge cleat. Injured was attempting to remove the rope from the cleat when it became taught, causing a sprained wrist.

Cleaning mud off the backstop of a conveyor with a sledge hammer and sprained left wrist.

Pulling suction hose out of barge and felt something pop in left wrist. Employee did not feel like EE needed treatment at the time. EE's wrist continued to bother EE so EE went in for initial diagnostic procedures on 6/29/2021. Employee was allowed to return to work with no restrictions. After an MRI employee was put on restricted duty as of 7/26/2021.

Employee was climbing in the loader on a snowy day and slipped on the ladder rung and fell to the ground. Employee sustained a fracture of the right hip.

Employee was trying to clear air lines that were plugged with material. EE was pulling on a hose to remove it when EE lost EE's footing and fell. When EE fell EE's left knee struck pipes that were on the ground. EE's fall resulted in a contusion on EE's shin and a possible meniscus tear in EE's left knee.

Employee suffered a strain to EE's left wrist while using a wrecking bar to check conveyor bearings and to remove built up material. The strain was caused by the repetitive twisting motion.

Employee fell down when he slipped on the ice/snow in the parking lot when he was getting out of his car to report to work. He sustained a contusion and sprain to his lower back.

Employee was done using a hand-held grinder. As he was switching hands to put it down, his index finger on his left hand was drawn in between the guard and the blade of the grinder which had not completely shut down. This caused a laceration to his finger which required stitches.

Worker's right hand was caught in a running machine which resulted in the loss of the tips of his 3 middle fingers and damage to his thumb. Worker did not follow proper lock out tag out procedure to ensure that the machine was locked out.

Employee's lower left leg was struck by a cable while moving a barge. Was first aid treatment until employee went to the doctor on 6/14. Injury is a bruise which was not drained but he was given work restrictions.

Employee was lifting port liners and a day or two later he noticed that he had a hernia. This was reported to management on 1/30/2012. Employee had been away on vacation for two weeks. Employee went to doctor on 1/30 and was diagnosed with a left direct inguinal hernia which will require surgery.

Employee climbed an extension ladder to tie it off and neglected to lock the top section of the ladder. The ladder slid down and the employee jumped off spraining his right ankle and foot.

Employee was using a rod to loosen material in a hopper. When the material broke free his arm came down and struck a metal plate that caused a laceration on his right forearm. Employee required 4 stitches to close the wound.

This injury was reported to us on 5/29/2008. Employee was repairing a pipe. When he was leaving, his right foot got caught between two pipes and he injured his right knee.

Employee suffered a heart attack while sitting in the breakroom. He was taken to the hospital by ambulance where he was pronounced dead.

Employee was doing general maintenance tasks in plant when he felt a burning twinge in his neck/shoulder area.

EE was installing a top to a journal when a finger on his left hand slipped under the bottom lip. His finger was cut and required stiches.

EE was using a skid steer to clean up snow in our parking lot when he struck what was left of a recently sawed off pipe with the bucket. When he hit the pipe he was thrown forward and struck the windshield with his hard hat. The main injury that he sustained was a cracked rib.

Employee was carrying a step ladder outside when he slipped on a snow covered rock and sprained his left ankle. An air cast was put on his ankle at the doctor's office.

Possible carpal tunnel. Employee is reporting that he has cramping in both wrists and hands. He feels that it is from repetitive use (shoveling, etc.) over a number of years. Even though he reported this on 11/1 he was just put on restrictions on 12/4/2006. Prior to this date there was no medical treatment involved.

Employee was running the skid steer inside the barge when he backed into the wall of the barge. He sustained injuries to his head, neck & shoulder when he struck the lap bar or the door bar.

When working at the dock ee felt a sharp pain in his left shoulder. It is suspected that he has a torn rotator cuff and he is on restricted duty.

Employee was climbing down into a barge on a ladder when he hit his right elbow on the track for the covers resulting in a laceration to his elbow.

One employee was moving a ladder when it fell and hit another employee on the right arm/shoulder. The injured employee sustained a contusion to his right shoulder.

Our employee was working on an installation of a catwalk when he strained his back.

Employee was pulling a garage door shut when the upper 2 panels fell off the track and struck him on the head & neck.

Employee was grinding metal with a portable grinder. A foreign object got in his left eye. He was wearing safety glasses.

Our employee sustained injuries to his nose & mouth during an altercation with another employee.

After attaching the "D" ring to the top of the skid steer, employee stepped off of the bucket and twisted his right ankle.

Employee was standing on the tires of a skid steer to remove the hooks from the pick points. His right foot slipped off of the tire and he fell and struck his right elbow on the frame of the the skid steer. He sustained a laceration on his right elbow that required 4 stiches.

This was a flare up of an old inury to employee's left elbow. The elbow swelled up and was locking. This report was filed late because it was initially thought that this would be a continuation of his previous injury. Our work comp provider has now told us that this is not the case.

Employee was carrying samples down from a platform when he tripped on the stairs and sustained a puncture wound to his right lower leg. He did not require medical treatment at that time. On 3/28/05 he noticed that the wound looked like it was infected. He went to the clininc and was given a prescription for antibotics.

Transferring filters to baghouse & one fell on his head

Employee was unjamming a motor with crank handle on the back of the motor when it spun around by itself striking him by his left eye.

Employee was carrying dust collector socks down a flight of steps when he slipped and fell down the stairs.

Employee was repairing a screw. He was pulling on the belts to get the screw to turn straining his back in the process.

The employee was helping to set barge covers. The crane operator moved the barge cover causing it to strike the employee. The employee was knocked down and suffered contusions to his left hand & wrist.

Employee walked through an area where steel grinding was being done. He felt an object go inside his safety glasses and into his eye. The object stuck in his eye causing an irratation

EE & A CO-WORKER WERE PUTTING THE COVER BACK ON THE HAMMERMILL WHEN EE PINCHED HIS FINGER.

EE WAS TURNING A PUMP HANDLE TO PUMP OIL FROM A DRUM. HE STRAINED MUSCLES IN HIS RT. RIB CAGE.

EE WAS WALKING THROUGH THE PLANT WHEN SOMETHING LODGED IN HIS RIGHT EYE CAUSING A CORNEAL ABRASION.

USING SCALER ON EQUIPMENT WHEN A METAL CHIP WENT INTO HIS RIGHT EYE.

EMPLOYEE WAS PUSHING ROCK IN A BARGE WITH SKID STEER, WHEN HE HIT A SEAMM WITH THE BUCKET CAUSING A SUDDEN STOP. EMPLOYEE STRAINED HIS BACK.

EE WAS CARRYING TOOLS AND PARTS UP STAIRS. STRAINED LOWER BACK.

EMPLOYEE WAS EMPTYING A GARBAGE CAN INTO THE DUMPSTER AND CAUGHT HIS LEFT HAND ON A SHARP EDGE OF THE DUMPSTER.

EE WAS CLIMBING UP A LADDER. THE LADDER SLIPPED FROM UNDER HIM CAUSING HIM TO FALL TO FLOOR AND SPRAIN HIS LEFT ANKLE.

EMPLOYEE WAS SETTING A FAN HOUSING IN PLACE USING RIGGING AND A FRONT END LOADER. WHEN GETTING IN THE LOADER, ANOTHER EMPLOYEE BUMPED THE STEERING WHEEL CAUSING THE BOOM TO MOVE. THE INJURED E MPLOYEE HAD HIS HAND ON THE FAN AND IT WAS PINCHED BETWEEN THE FAN AND A BEAM.

EE WAS SHOVELING MATERIAL AND STRAINED HIS BACK

EMPLOYEE WAS BENDING AND TWISTING WHILE REPLACING A PUMP FILTER.

EE SLIPPED WHILE CLIMBING DOWN A LADDER. HE FELL FORWARD CHIPPING A FRONT TOOTH ON A RUNG.

EE WAS SHOVELING MATERIAL AND PUSHING WHEELBARROW, CAUSING A WRIST STRAIN.

EE WAS PULLING UP VACUUM HOSE CAUSING A BACK STRAIN.

EE WAS TRYING TO TIP A BARREL BY LIFTING IT FROM BEHIND. IN DOING SO HE STRAINED HIS LOWER BACK.

EMPLOYEE WAS USING A UTILITY KNIFE AND LACERATED HIS FINGER.

EE WAS USING A ROD TO BREAK APART MATERIAL IN A BIN WHEN A ROCK FLEW UP INTO HIS FACE, HITTING HIS FRONT TOOTH WHICH KNOCKED HIS CAP LOOSE.

EE WAS UNPLUGGING A PIPE. WHEN HE PULLED THE PIPE APART PRESSURE FOREHAD MATERIAL OUT AND INTO HIS FACE/EYE.

WORKING ON EQUIPMENT WHEN SOMETHING GOT IN EMPLOYEE'S EYE.

EMPLOYEE WAS CLIMBING OUT OF THE BARGE WHEN THE LADDER SLIPPED AND THE EMPLOYEE AND THE LADDER FELL TO THE BOTTOM OF THE BARGE. EMPLOYEE INJURED HIS HEAD, BACK AND ANKLE.

EE AGGREVATED A TENDONITIS PROBLEM IN HIS ELBOW FROM REPETITIVE ACTIVITY RELATED TO BAGGING PRODUCT.

EE STRAINED LOWER BACK WHILE WELDING IN A TIGHT CONFINED AREA.

EE WAS MOVING A DRUM. ABDOMEN STRAIN.

EE WAS REMOVING A COVER FROM A COMPRESSOR. INJURED LEFT ELBOW.

EE WAS ROBBING MATTERIAL OUT OF A BIN WHEN WAS HAND STRUCK THE BIN. HIS RIGHT HAND WAS BROKEN.

EE CALLED IN SICK ON 2/28 WITH EYE PROBLEM PINK EYE. WORKED ON 3/1 BUT REPORTED THAT DUST HAD AGGRAVATED THE BAD EYE. WENT TO CLINIC AND WAS DIAGNOSED WITH CORNEAL ABRASON. THIS INJURY WAS NOT THOUGHT TO BE WORK RELATED AT FIRST. U

LACERATED RIGHT RING FINGER WHILE REMOVING A BEARING.

POUNDING ON A BIN GATE TO CLOSE IT. SUSTAINED A CUT ON HIS LEFT HAND WHEN THE MAUL SLIPPED.

EMPLOYEE WAS CHANGING THE SETTING FOR A FAN DAMPER WHEN IT CAME DOWN ON HIS WRIST.

EE WAS SHOVELING MATERIAL WHEN HE STRAINED HIS BACK. HE DID NOT GO IN FOR TREATMENT UNTIL A LATER DATE.

EMPLOYEE WAS LIFTING DOCK PLATE WHEN HE STRAINE DHIS BACK.

EE WAS BENDING OVER TO PICK UP A VALVE TO TAKE UP A LADDER. AS HE WAS PICKING UP THE VALVE ANOTHER EE COMING DOWN THE LADDER ACCIDENTALY STEPPED ON HIS HEAD CAUSING HIS HEAD TO HIT THE VALVE THIS CAUSED A LACERATION ABOVE HIS RIGHT EYE REQUIRING 8 STITCHES

WHILE PICKING UP A PIECE OF EQUIPMENT THE EMPLOYEE STRAINED HIS RIGHT SHOULDER.

EMPLOYEE WAS CLIMBING DOWN AN EXTENSION LADDER. THE LADDER SUPPORTS FOULED AND THE EMPLOYEE AND THE LADDER FELL AGAINST THE BIN IT WAS AGAINST. THE EMPLOYEE SUSTAINED A BRUISED AND SPRAINED KNEE.

EE WAS ARRANGING 50 LB BAGS OF MATERIALS.

EMPLOYEE WAS BENDING OVER USING A TRUCK VACUUM HOSE WHEN HE SUFFERED A BACK STRAIN.

EE WAS RE-ARRANGING A BAG ON THE PALLETIZER CONVEYOR. HE HAD ONE FOOT ON THE BELT WHEN ANOTHER EE STARTED & STOPPED THE BELT. THE OTHER EE THOUGHT THAT HE HAD HEARD HIM WHEN HE TOLD HIM HE WAS GOING TO START THE BELT.

EE WAS CHECKING A COUPLING ON A LP HOSE WHEN HE ACCIDENTALLY SHOT PROPANE INTO HIS LEFT EYE.

EE WAS REMOVING A MOTOR. HE WAS USING A UTILITY KNIFE TO CUT ELECTRICAL TAPE. THE KNIFE SLIPPED AND HE CUT HIMSELF ON THE FOREARM.

EMPLOYEE WAS HANDLING A PALLET WHEN HIS FINGER BECAME STUCK BETWEEN THE BOARDS.

LIFTING A 4-WAY PALLET.

AFTER WORKING ON A MAINTENANCE PROJECT HE NOTICED HE HAD SOMETHING IN HIS EYE.

THE INJURY WAS NOT DUE TO A SPECIFIC EVENT, JSUT A GENERAL SORENESS IN EE'S LEFT SHOULDER. HE DOES DO SIGNIFICANT LIFTING OF 50# BAGS.

HE WAS WELDING WHEN SOME SLAG GOT INTO HIS RIGHT EYE.

EE BENT OVER TO TAKE A GUARD THAT WAS BEING HANDED UP TO HIM. HE FELT PAIN IN HIS LOWER BACK WHEN LIFTING THE GUARD. EE DID NOT REPORT THIS ACCIDENT. WE ONLY BECAME AWARE OF IT ON 1-27-97, WHE N THE CLINIC THAT HE WENT TO FOR TREATMENT CALLED REGARDING PAYMENT.

EE HAD FINISHED CUTTING A BEAM. HE REMOVED HIS FACE SHIELD AND WHILE CLIMBING DOWN A LADDER, SOMETHING LODGED IN HIS EYE.

EE WAS RODDING OUT A HOPPER WHEN HIS LEFT RING FINGER GOT CAUGHT BETWEEN THE ROD AND THE HOPPER.

EMP REPORTED THAT HE FELT PAIN IN ELBOWS ON 4-30WHILE BAGGING PRODUCT. NOTIFIED THE OFFICE OF THE PROBLEM ON 5-14. STRAIN SEEMS TO BE CAUSED BY PERFORMING BAGGING OVER TIME AND NOT BY ONE INCI DENT.

EMPLOYEE WAS EMPTYING A BIN OF VARIOUS WASTE MATERIALS WHEN A PARTICLE BECAME LODGED IN HIS LEFT EYE.

HE WAS CUTTING WITH A TORCH ON THE ROCK ELEVATOR. IT WAS VERY WINDY AND PARTICLES BLEW INTO HIS EYE FROM UNDER HIS SAFETY GLASSES.

EE WAS WELDING HAMMERS; WHEN HE WAS CLEANING THEHAMMERS, A CHUNK OF SLAG FLEW INTO HIS RIGHT EYE

EE WAS WALKING OUT OF THE PLANT WHEN A PIECE OF ICE WHICH HAD FALLEN FROM THE ROOF, STRUCK HIM IN THE HEAD. HE WAS WEARING A HARD HAT. THE BLOW KNOCKED HIM TO HIS KNEES. WE ARE CURRENTLY CONST RUCTING A CANOPY OVER THE AREA TO PREVENT THIS FROM HAPPENING AGAIN.

EMP WAS STEPPING FROM A LADDER ONTO A LEDGE ON AMINERAL TANK. HE SLIPPED ON THE LEDGE FALLING ABOUT 4' TO THE FROUND. THIS TANK IS OUTSIDE AND IT HAD BEEN RAINING ALL MORNING. SUSTAINED BRUISE D LEFT KNEE.

EE WAS OPERATING A HAND HELD GRINDER. WHEN HE SET THE GRINDER DOWN, THE WHEEL TOUCHED THE INSIDE OF HIS WRIST.

EE STATES THAT HIS CONDITION WAS CAUSED BY REPEATEDLY BUMPING HIS ELBOW WHILE WORKING DURING THE PAST TWO YEARS.

WHILE WORKING ON A BLOWER, EE TRIED TO RETRIEVE A BOLT THAT HAD FALLEN INSIDE. HIS FINGER WAS CUT BY ONE OF THE BLADES.
