Injured was reinstalling Raymond mill panels using a 2-ton overhead chain hoist. The chain hoist trolley over-traveled the end of the beam and fell, striking injured on side of head and shoulder, causing EE to strike EE's face on the mill structure.

Injured was assisting a barge movement when a rope became pinched in a barge cleat. Injured was attempting to remove the rope from the cleat when it became taught, causing a sprained wrist.

Pulling suction hose out of barge and felt something pop in left wrist. Employee did not feel like EE needed treatment at the time. EE's wrist continued to bother EE so EE went in for initial diagnostic procedures on 6/29/2021. Employee was allowed to return to work with no restrictions. After an MRI employee was put on restricted duty as of 7/26/2021.
Employee was climbing in the loader on a snowy day and slipped on the ladder rung and fell to the ground. Employee sustained a fracture of the right hip.

Employee was trying to clear air lines that were plugged with material. EE was pulling on a hose to remove it when EE lost EE's footing and fell. When EE fell EE's left knee struck pipes that were on the ground. EE's fall resulted in a contusion on EE's shin and a possible meniscus tear in EE's left knee.

Employee was done using a hand-held grinder. As he was switching hands to put it down, his index finger on his left hand was drawn in between the guard and the blade of the grinder which had not completely shut down. This caused a laceration to his finger which required stitches.

Worker's right hand was caught in a running machine which resulted in the loss of the tips of his 3 middle fingers and damage to his thumb. Worker did not follow proper lock out tag out procedure to ensure that the machine was locked out.
Employee's lower left leg was struck by a cable while moving a barge. Was first aid treatment until employee went to the doctor on 6/14. Injury is a bruise which was not drained but he was given work restrictions.

Employee was lifting port liners and a day or two later he noticed that he had a hernia. This was reported to management on 1/30/2012. Employee had been away on vacation for two weeks. Employee went to doctor on 1/30 and was diagnosed with a left direct inguinal hernia which will require surgery.

Even though he reported this on 11/1 he was just put on restrictions on 12/4/2006. Prior to this date there was no medical treatment involved.

This was a flare up of an old inury to employee's left elbow. The elbow swelled up and was locking. This report was filed late because it was initially thought that this would be a continuation of his previous injury. Our work comp provider has now told us that this is not the case.

On 3/28/05 he noticed that the wound looked like it was infected. He went to the clininc and was given a prescription for antibotics.

EE BENT OVER TO TAKE A GUARD THAT WAS BEING HANDED UP TO HIM. HE FELT PAIN IN HIS LOWER BACK WHEN LIFTING THE GUARD. EE DID NOT REPORT THIS ACCIDENT. WE ONLY BECAME AWARE OF IT ON 1-27-97, WHE N THE CLINIC THAT HE WENT TO FOR TREATMENT CALLED REGARDING PAYMENT.

EE WAS WALKING OUT OF THE PLANT WHEN A PIECE OF ICE WHICH HAD FALLEN FROM THE ROOF, STRUCK HIM IN THE HEAD. HE WAS WEARING A HARD HAT. THE BLOW KNOCKED HIM TO HIS KNEES. WE ARE CURRENTLY CONST RUCTING A CANOPY OVER THE AREA TO PREVENT THIS FROM HAPPENING AGAIN.

EMP WAS STEPPING FROM A LADDER ONTO A LEDGE ON AMINERAL TANK. HE SLIPPED ON THE LEDGE FALLING ABOUT 4' TO THE FROUND. THIS TANK IS OUTSIDE AND IT HAD BEEN RAINING ALL MORNING. SUSTAINED BRUISE D LEFT KNEE.
