HE LIFTED HIS WELDING HOOD TO GET A NEW ROD, HEARD A SLEDGE HAMMER STRIKING AND FELT SOMETHING HIT HIS RIGHT CHEEK. MEDICALTREATMENT WAS SOUGHT FOR THE FIRST TIME ON 4/20/94 - UP UNTIL THEN NOTHING OF ANY KIND OF FIRST AID HAD BEEN DONE OR REQUESTED BY EMPLOYEE.

WHILE OPERATING THE BULLDOZER AT THE SCIOTO VALLEY RESOURCES DIVISION IN LUCASVILLE, OHIO, STRIPPING DIRT. I WAS OPENING A DRAINAGE DITCH, WHEN I BACKED UP INTO A 10 FOOT HOLE THAT WAS COVERED WITH WATER. THE DOZER ENDED UP IN A NEAR VERTICAL POSITION, JARRING BACK WINDOW FRAME AND STRIKING ME IN BACK.
