Melvin Stone Co. LLC · Fall from machine

Conveyor went down due to being overloaded and they commenced to clearing the belt off to get back running when employee was repositioning feet they caused them to fall down on the rock surface below conveyor landing on their back onto the loose rocks. CT scans and x-rays determined they had compound fractures in their T1, T11 and T12 discs in the back along with a bruised lung.

Melvin Stone Co. LLC · Caught in, under or between a moving and a stationary object

Employee had locked and tagged out the crusher to adjust the curtain. The rotor was on the "up-stroke" and the employee pushed the rotor over with their hand rather than using a board or steel bar and EE's glove got caught between the rotor and housing. This caused a cut to EE's right thumb requiring 6 stitches.

The employee was cleaning the return roller on the crusher. Employee's hand and arm became caught between the roller and belt.
