Sharps Quarry & Plant: 1 recorded fatality Metal/Non-Metal

Employee walked over to the mobile stacker to reach for a shovel while the stacker was being repositioned. The operator did not have eye contact with the employee at the time as the employee went to grab the shovel, the track moved and caught EE's left foot. The travel alarm was working.

(Change injury status from first aid to recordable 9-10-24) On 8-30-24 Employee was getting into EE's service truck; EE heard a pop sound in EE's left knee followed by some mild pain. On 9-10-24 EE re-examined by the doctor and referred to Physical therapy resulting in a first aid medical case to recordable injury.

Employee was removing loose rock that got lodge in the V-belts/gear box on cv02, Employee was holding onto the v-belts with right hand and removed loose rock with the left hand. When the rock came loose, the conveyor belt rolled backwards, this resulted in right middle finger to get pinched in between the v-belts and the motor pulley.

Employee was using a retractable safety utility knife to cut a piece of flashing and the knife blade failed to retract back like it is designed. Resulting in a laceration on the underside of their forearm requiring 5 stitches.

Employee was working in feeder area. Employee lost footing, fell forward, and struck forearm, causing a fracture.

Employee had removed his safety glasses and had dust blown into his right eye causing a corneal abrasion that required a prescribed antibiotic ointment.

Employee tried to drive a broom under a low clearance bar, causing the roof of the broom to break loose and strike the employee on the left index finger, and on the head. The left index finger required two stitches.

Employee was wearing eye protection while welding and grinding. At some point during the day metal dust fell into employee's eye in which he began to rub it due to irritation causing metal to imbed. Employee was taken to medical clinic to have metal removed from eye. He returned to work that same day without restrictions.

Employee received minor lower back strain when his truck bed of end-dump collided with conveyor belt brace.

EE was working from an aerial lift and as he activated the basket, it moved and he lost balance, falling next to basket rails. He strained his lower back. No time lost due to this incident. He returned to work the next day.

Employee was operating a Bobcat skid steer loader, cleaning out under a Simplicity screen, when a large rock came out of the screen chute and fell inside the safety cab of the Bobcat loader and struck him on the inside of his left shin. Result : a deep bruise on left shin. The rock was approx: 12 inches by 4 inches.

While attempting to lift and load shaker screens from a storage rack, the storage rack broke at a weld point and caused the other screens to fall over striking and pushing down employee, resulting in a sprained left ankle.

Employee and a co-worker were dismantling/cutting up an old metal catwalk which was supported by a crane with chain slings and an additional chain to prevent excessive swing. The employee cut the last piece of metal; it swung free and hit him in the back of his head and knocked him down. He was assisted immediately and didn't think he needed a doctor.

EE WAS REMOVING A DRIVE BELT FROM THE CRUSHER JAW, WHILE PULLING DRIVE BELT LOOSE FROM THE GUARD, HIS LEFT FOOT WAS ON SLOPING PORTION OF THE CATWALK,PERPENDICULAR TO SLOPE. HIS ANKLE ROLLED OVER & HE IMMEDIATELY FELT IT. HE REPORT TO FILL IN FORMAN BUT DIDN'T FEEL NEED FOR MEDICAL HELP UNTIL FOLLOWING AFTERNOON, WHEN EXPERIENCED SWELLING PAIN

EE WAS INVOLVED IN THE PROCESS OF REFILLING MULTIPLE TYPES OF OILS ON THE SERVICE TRUCK FROM 55 GAL. OIL DRUM LOCATED ON THE GROUND. THE DRUMS HAD TO BE MOVED IN ORDER TO BE CLOSE ENOUGH TO SERVICE TRUCK FOR TRANSFER OF OILS. THE MOVEMENT WAS BEING ACCOMPLISHED MANUALLY WHEN THE STRAIN TO EE'S LOWER BACK OCCURRED.

ON 3/1/04 @ SHARPS QUARRY, EE WAS HAULING OVERBURDEN AS PART OF STRIPPING PROCESS. DURING HAULING HE TRAVELED THROUGH A MUD PUDDLE, THERE WAS A SOFT SPOT, WHICH CAUSED ONE OF THE TIRES TO DIP WHICH CAUSED THE TRUCK TO JERK. THIS JERKING ACTION CAUSED EE TO BE WHIPPED IN OPERATOR'S SEAT WHILE BELTED, CAUSING THE STRAIN TO LOWER BACK. EE DECLINED TO GO TO DOCTOR UNTIL 6/17/04.

EMPLOYEE WAS CHANGING OUT HEAVY ROLLERS ON A CONVEYOR WHEN HE TWISTED HIS RIGHT WRIST.

EE WAS HELPING MECHANIC, MAN UNPIN THE HORSE BACKHOE ATTACHMENT AT THE JAWS INORDER TO REMOVE IT. HE GRABBED A BRACE TO ADJUST HIS FOOTING AT THE SAME TIME OTHER HIT THE BRACE WITH A 3 LBS. SH OP HAMMER, SMASHING HIS THUMB LACERATION IT. HE WAS THEN ADMINISTERED FIRST AID BY HIS SUPERVISOR OTHER BY WRAPPING THE HAND AND THUMB AND APPLYING ICE. THEN TRANSPORTED TO CLINIC.

EE WAS STANDING ON RADIAL STACKER CONVEYOR. HE PULLED ON WINCH CABLE AND SLIPPED AND FELL.

A DRIVER FELL FROM A TRUCK 1 TON. IN HIS ATTEMPT TO BREAK HIS FALL, HE FRACTURED HIS LEFT WRIST.

INJURED WAS ON HIS BACK PUSHING A ROCK OFF WITH HIS RIGHT FOOT, ROCK FELL OUT OF TRACK AND LANDED ON HIS LEFT FOOT.

EMPLOYEE WAS CLIMBING ON LOADER WHEN HIS FOOT SLIPPED, CAUSING HIM TO STRAIN HIS UPPER BACK.

INJURED ATTACHED CHAIN TO ANVIL RING (PART OF CRUSHER), AS I WAS BEING LIFTED OUT, RING BROKE & CHAIN HIT KENNY IN THE FACE

EE WAS STANDING ON TRAC OF D-8 DOZER TALKING TO ANOTHER EMPLOYEE. HE TURNED TO GET BACK INTO CAB & SLIPPED & FELL STRIKING HIS HIP ON TRAC AND GROUND. WENT TO DOCTOR 3-30-93.

HE WAS CHANGING DRILL STEEL FOR I.NAME. I.NAME HAD NOTICED THAT THE STEEL HAD BROKEN IN THE COLLAR INSTEAD OF THE SHANK. I.NAME REACHED FOR THE STEEL WITH THE STEEL HOLDER TOOL AT THE SAME TIM E INJURED WAS REACHING FOR THE STEEL. THE STEEL TOOL STRUCK HIM ON BOTH HANDS. HE LOST HIS LITTLE FINGER ON RIGHT HAND AT 1ST JOINT.

INJURED WAS HELPING PUT TEETH ON LOADER BUCKET, ONE TOOTH WAS STUCK, SO HE HAD ANOTHER EMPLOYEE HIT IT WITH A SLEDGE HAMMER AND IT SPLINTERED STICKING HIM IN THE UPPER LIP.

EMPL PUNCHING R2CK FROM HOPPER WITH BAR WHEN LITTLE FINGER OF LEFT HAND STRUCK EDGE OF HOPPER AND GOT CAUGHT BETWEEN BAR AND HOPPER CAUSING SKIN TO TEAR.

EMPLOYEE WAS LIFTING ROCKS FROM SECONDARY CRUSHER AND TURNING HANDING THEM TO ANOTHER MINER. UPON LIFTING AND TURNING WITH A ROCK OF APPROX. 10 LB. EMPLOYEE STRAINED HIS BACK.

I NAME WAS REMOVING BOLTS FROM SECTION OF CONVEYOR OF ASPALT PLANT BEING CONSTRUTED. THE CONVEYOR SWUNG FREE AT WHICH TIME MINER GRABBED CONVEYOR TO PROTECT ALLOW MINER. IN DOING SO MINER THU MB WAS PINNED BETWEEN ANOTHER SECTION OF CONVEYOR MINER RIGHT THUMB WAS BROKEN AT FIRST JOINT FROM HANDS OF CONVEYOR MINERS RIGHTG THUMB WAS BROKEN AT FIRST JOINT FROM HANDS THERE WAS NO LOSS
