EMPLOYEE WAS SENT TO SHARPS QUARRY TO SERVICE A CAT 621 SCRAPER FOR MCCLINTON-ANCHOR COMPANY.HE WAS OPERATING A 1989 FORD F700 WITH A HIAB 173 SPEED LOADER BOOM HOIST WHEN THE ACCIDENT HAPPENE D.HE WAS USING THE HOIST LIFTING A 29.5X35 (APPROXIMATELY 900 POUND) TIRE OFF THE REAR OF THE SCRAPER. WITNESSES AT THE SCENE REPORTED HE HAD JUST CHANGED THE RIGHT REAR TIRE AND HAD MOVED TH

Employee walked over to the mobile stacker to reach for a shovel while the stacker was being repositioned. The operator did not have eye contact with the employee at the time as the employee went to grab the shovel, the track moved and caught EE's left foot. The travel alarm was working.

(Change injury status from first aid to recordable 9-10-24) On 8-30-24 Employee was getting into EE's service truck; EE heard a pop sound in EE's left knee followed by some mild pain. On 9-10-24 EE re-examined by the doctor and referred to Physical therapy resulting in a first aid medical case to recordable injury.

Employee was removing loose rock that got lodge in the V-belts/gear box on cv02, Employee was holding onto the v-belts with right hand and removed loose rock with the left hand. When the rock came loose, the conveyor belt rolled backwards, this resulted in right middle finger to get pinched in between the v-belts and the motor pulley.

Employee was wearing eye protection while welding and grinding. At some point during the day metal dust fell into employee's eye in which he began to rub it due to irritation causing metal to imbed. Employee was taken to medical clinic to have metal removed from eye. He returned to work that same day without restrictions.

EE was working from an aerial lift and as he activated the basket, it moved and he lost balance, falling next to basket rails. He strained his lower back. No time lost due to this incident. He returned to work the next day.

Employee was operating a Bobcat skid steer loader, cleaning out under a Simplicity screen, when a large rock came out of the screen chute and fell inside the safety cab of the Bobcat loader and struck him on the inside of his left shin. Result : a deep bruise on left shin. The rock was approx: 12 inches by 4 inches.

Employee and a co-worker were dismantling/cutting up an old metal catwalk which was supported by a crane with chain slings and an additional chain to prevent excessive swing. The employee cut the last piece of metal; it swung free and hit him in the back of his head and knocked him down. He was assisted immediately and didn't think he needed a doctor.

EE WAS INVOLVED IN THE PROCESS OF REFILLING MULTIPLE TYPES OF OILS ON THE SERVICE TRUCK FROM 55 GAL. OIL DRUM LOCATED ON THE GROUND. THE DRUMS HAD TO BE MOVED IN ORDER TO BE CLOSE ENOUGH TO SERVICE TRUCK FOR TRANSFER OF OILS. THE MOVEMENT WAS BEING ACCOMPLISHED MANUALLY WHEN THE STRAIN TO EE'S LOWER BACK OCCURRED.

ON 3/1/04 @ SHARPS QUARRY, EE WAS HAULING OVERBURDEN AS PART OF STRIPPING PROCESS. DURING HAULING HE TRAVELED THROUGH A MUD PUDDLE, THERE WAS A SOFT SPOT, WHICH CAUSED ONE OF THE TIRES TO DIP WHICH CAUSED THE TRUCK TO JERK. THIS JERKING ACTION CAUSED EE TO BE WHIPPED IN OPERATOR'S SEAT WHILE BELTED, CAUSING THE STRAIN TO LOWER BACK. EE DECLINED TO GO TO DOCTOR UNTIL 6/17/04.
