Employee was at work, sitting in a truck, when they started having chest pains. The supervisor felt they needed to get checked out and drove employee to the doctor. After examination, medical staff decided employee need further care and sent them to the hospital. No injury attributed to the condition.

Employee was tasked with climbing into a screen deck to perform maintenance. When the employee climbed in EE complained that EE hurt shoulder. Employee was taken to the doctor and was treated as a first aid case. On 11/2/2016 the case became reportable due to a second doctor's opinion saying that the employee needed surgery on the shoulder.

Employee was dragging a piece of 8" PVC tubing attempting to attach it to the water pump on a barge and stepped backwards into a gap between the barge and catwalk. He fell down and struck the backside of his left upper arm on the corner of the handrail which caused a laceration that required five stitches.
End dump driver was backing under pit loader, pit loader said his bucket dropped, hitting bed of truck and causing haul truck to come off ground. When pit loader raised bucket up, haul truck came down, causing it to jump forward causing employee's neck to move suddenly. Haul truck driver was wearing seat belt.

HE CALLED IN M ONDAY, 8-14-00 ABOUT INJURY TO ANKLE; WOULD NOT BE TO WORK. NO MENT THAT IT WAS WORK RELATED. RETURNED TO WORK FOR 12 1/4 HOURS 8-17-00, AND HAS NOT WORKED SINCE.
WHILE BACKING LOADER DOWN THE RAMP, THE LOADER CAME UPON AN UNEXPECTED ROUGH SPOT PRODUCING UNEXPECTED MOTION. HE WAS LOOKING BACKWARD AND AS A RESULT OF THE UNEXPECTED MOVEMENT, HE HIT HIS MO UTH ON THE WINDOW LATCH AND BROKE A TOOTH.
