OPERATING WAS O1ERATING A TS24 PAN. HE CAME OUT OF PIT WITH A LOAD OF DIRT ONTO THE FILL. HE GOT TO CLOSE TO EDGE OF FILL CAUSING PAN TO ROLL ON ITS LOST SIDE. OPERATOR REPORTED HE WAS NOT INJ URED AT THAT TI4E OF ACCIDENT. DID NOT FILE FOR ANY BENEFITS UNTIL 7-17-86.

EE WAS ATTEMPTING TO HOOK PIPE ON DOZER BLADE HE WAS HAVING TROUBLE HOOKING CHAIN WHEN OPERATOR LOWERED BLADE TOG ROUND. BLADE WAS ABOUT 5" FROM GROUND. EES LEFT FOOT WAS UNDER BLADE. THE OP ERATOR THOUGHT EE WAS IN THE CLEAR OF BLADE.
