August 12, 2019 NC · Metal/Non-Metal bulldozer operator, universal operator, heavy equipment operator, operating engineer POWERED HAULAGE
Shelter Creek Capital, LLC · Struck against a moving object

Miner informed supervisor that they had bumped their head on the windshield of the bulldozer and cracked the glass when the dozer lunged forward as a result of the unit going into park and the seat belt not holding. The individual stated that they had a small bump on their head but was not hurt.

Shelter Creek Capital, LLC · Struck against a moving object

On 8/15/2019 miner told manager that their back had been hurt during the week of 7/12/19 when the off road truck they were operating hit a hole and they were thrown against the armrest. Injured had told supervisor on or about 7/8/19 that they had hurt back away from work and that they could not run a off-road truck because it hurt their back after that.
