A non-injury roof fall was discovered in the intake airway near spad 15550 which measured approximately 20 feet long x 20 feet wide x 10 feet high. All approaches were dangered off and breaker posts were set.

A roof fall was discovered in the primary escapeway near spad 15550 on No. 5 Belt at No 18 Break. Work crews were withdrawn to an area outby the fall. Fall measured approximately 40 feet long x 20 feet wide x 10 feet high.

The employee continued to work and reported to the doctor on 9/11/2018. After and x-ray was performed, it identified a minor fracture to the left index finger.

Employee had continued pain and followed up with employee's physician, a torn meniscus was discovered. The employee was take off of work for surgery 6/20/2017. This will also be the first day missed since the accident.

EE was operating a scoop in preparation for construction of a permanent ventilation control in the #9 entry. While operating the scoop, a broken piece of ventilation control, known as a pogo stick entered the operator's deck of the scoop striking the EE in the face near EE's right eye. Causing a fracture to EE's nose and allowed debris to enter right eye resulting in a scratch.

Passage of persons was affected, ventilation was not affected, fall area will be rehabilitated.

Employee was kneeling down removing rock out of the walkway, when he felt a pop in his knee. The employee returned to work, and worked until 7/20/14. He returned to the doctor with continued pain in his left knee. The doctor took him off work on 7/21/14.

Was using a crib block to drive an axle in the #2 roof bolter, when he hit the axle with the crib block it bounced back striking him in the mouth. This resulted in a split lip that required 27 sutures.

Floor was wet and muddy, while carrying roof bolts plates to supply bolter. Employee slipped and smashed finger between roof bolt plates and bolter. This resulted in a 3/4" laceration to left middle finger that required four sutures.

While traveling down track, mantrip came off track. While jacking up ride, EE could not get leverage with jack bar so he started using slate bar. He thought he heard the jack "click" lock into place so he took his hand off of the slate bar. It swing up striking him in the hand and also hitting him in the face. Resulted in bruised hand & 3/4" laceration requiring 6 sutures.

Employee was taking his glove off to eat lunch. His glove got caught on an inner latex glove worn to prevent chafing. When the glove caught, it jarred his hand into a stack of roof plates causing a laceration on the back of his left hand. The laceration was deep enough for 3 sutures. Note: the employee did finish his regular shift and then went to doctor.

No injuries or entrapment involved. No escapeways, equipment or ventilation effected. This is an abandoned panel and the #6 entry is not traveled.

No injuries or entrapment was involved. The Fletcher Roof Bolter was covered by the fall. The entry travelway was involved in fall and ventilation to face involved. No escape way was affected.

Member was helping install a stopping. He was throwing blocks over to Members that were constructing stopping, when he turned to throw one he caught his left little finger between block in his hand and the stopping being built, resulting in a fracture to his little finger on the left hand. The finger was splinted.

The pipe popped loose striking the slate bar which resulted in the bar striking his finger. The result was a dislocated finger with medical treatment only of a splint applied. No days lost.

While loading a fusion machine into the bed of a pickup truck, EE was lowering the handle of the machine, causing it to pinch his left middle finger. The injury to his finger required part of the distal phalange to be amputated. This accident was not received at Marfork until 10-23-2009

EE & a co-worker were building a stopping in the #2 entry, the seam height is 10'4" at this location and is on a hill. The right side of the stopping was stacked almost high enough to wedge. EE climbed down from the ladder to retrieve wedges when the stopping fell, striking him on the right hand.
