Employee was climbing the stairs and as EE attempted to step up EE's foot slipped causing EE's weight to be completely supported by EE's arms resulting in EE dislocating EE's right shoulder.

EMPLOYEE WAS MOVING IMPACT ROLLERS ON THE CONVEYER AT THE JAW. HE CUT LOOSE THE BOLTS ON HIS SIDE. HE TURNED OFF THE TORCH AND HANDED IT TO OTHER EMPLOYEE. ACETYLENE LINE BURST SHOOTING FLAMES FROM THE TORCH UNDER HIS GLOVE. IT BURNT THE TOPOF HIS LEFT HAND AND THUMB.

THE CUTTINGS FROM THE SAW WERE VE RY FINE AND THE WIND WAS BLOWING. THE GLASS IN THE SWITCH HOUSE HAD BEEN REMOVED SO THE BEAM COULD BE CUT. THE WIND BLEW A SMALL PIECE OF METAL INTO HIS LEFT EYE.
