South Hallsville No 1 Mine Coal

Employee was replacing an axle shaft of Komatsu HD1500-7 Water Truck. Employee found the axle bushing was stuck and used a glove hand to attempt to lift the sun gear side of the axle to free it. Upon pushing upwards on the sun gear, the employee lost grip on the oily surface and EE's forearm contacted a spline on the sun gear resulting in a 1.5" laceration requiring 5 stitches.

A grass fire started in the old Q-area, cause is unknown. there is no mining activity in the Q-area. the local fire department was on scene and got the fire under control.

Two employees were at the coal barn installing a wing shroud (steel plate) on a dragline bucket. The upper piece was set in place and the crane was lowered. Both employees were going to adjust the piece of steel plate by hand when it fell. It slid down the side of the bucket, hit the ground and fell over hitting the employee breaking two Metatarsal bones in EE's right foot.

While setting up to change the hoist ropes on 102 dragline a pin that secures the shackle of the snatch block was not screwed in. This resulted in the shackle spreading apart and the snatch block popping out hitting the employee in the leg breaking the left tibia.

Employee was replacing a 2' x 4' x 3/8" piece of steel plate on a bulldozer blade. The employee stood the plate up resting the 2' edge on the floor and begin walking the plate corner to corner to the center of the blade. Once they got to the center of the blade they began laying the plate down on its opposite edge (4' side)when the injured employee felt a pull in left arm.

While on a break from ERT training, two employees were looking at a pocketknife. The injured employee made a motion to point out a function of the knife at the same time that the employee holding the knife happened to flip the blade open. The knife blade cut the injured employees right index finger and sutures were required to close the laceration.

While miner was waiting for polymer to finish transferring from trailer to stationary pre treatment tank, miner turned to the right and miner's feet slipped in the wet mud causing miner to fall and fracture right leg. The wet, muddy, red clay made very slippery conditions.

Employee stepped onto a berm to observe the cut conditions. The injured employee turned to walk down the berm and stepped into a low spot in the berm. The injured employee lost balance after stepping in the low spot and fell to the ground. The result of the fall was a dislocated right ankle and a broken right fibula bone.

While finishing drag rope change out on a 1570 dragline. The winch line dropped out of the horizontal fairlead sheave, employee was guiding the cable back into the sheave. During this process employee's left thumb was pinched between the outer and inner horizontal sheaves fracturing the thumb. The employee was wearing leather gloves at the time of the incident.

While attempting to step across the hood opening on 728 end dump, the employee stepped short and fell on EE's right side, mid-torso. The fall resulted in two broken ribs.

Worker obtained a laceration to the left 5th digit while cutting a metal plate. EE was wearing all appropriate PPE including leather gloves. EE did receive sutures.

The employee was cleaning pond over flow drain. The pipe inlet has an expanded metal barrier to catch large debris from entering the inlet. The employee was reaching down to remove large debris from barrier and was bit on EE's right index finger by a venomous snake hiding under the debris. The worker was wearing rubber style gloves when bit.

The employee lost grip on the left grab iron before foot was on the ground resulting in a fall backward. The employee's head hit the bottom part of the ripper frame, which cut the top of head.

A grass fire started in the old B-Area, caused when a hay baler caught fire. The fire was contained with 15-20 min, but the hay bales within the burnt area were still on fire.

A grass fire started in the old E-area, the cause might have been a lightning strike. There is no mining activity in the E-area. The local fire department was on scene and got the fire under control.

Employee was flipping cutting edges on a D-11 bulldozer. The employee was using a 24-inch pry bar to rotate the cutting edge. When the cutting edge unexpectedly shifted employee got right pinky finger pinched between the pry bar and the shop floor. Employee was wearing leather gloves.

Employee pinched finger between synthetic sling and jib crane hook.

Employee was replacing the wheel studs on 605 fuel/lube truck, using a punch and hammer. Employee was holding the punch with gloved right hand when the hammer glanced off the punch striking EE's thumb.

EE was replacing the idler arms on a D10 dozer. EE was using a RAD torquing wrench on inside bolt. The reaction arm was pointing upward. EE placed right hand on reaction arm & left hand on the trigger of the wrench. When EE applied torque pressure to the wrench, the reaction arm rotated causing right hand to be caught/pinched between the idler & the reaction arm.

Using a pry bar to remove Becket from Dump Rope Socket. Pry bar slipped, smashing employee's thumb between pry bar and dump rope sheave. An x-ray showed a fracture of the tip of employees left thumb. The employee was wearing gloves at the time of this incident.

Employee opening the rear doors on a 53' tool trailer to gain access to the inside. Employee had release and pulled on the inner latch handle and was attempting to unlatch the outer latch when the handle came toward employee striking employee in the upper right lip. Employee received 8 sutures in upper lip. No equipment damage.

EE fell in restroom, hit their head on a cabinet and caused a bump and minor cut that stopped bleeding without a bandage. As a precautionary measure, 911 was called. Due to personal medical reasons, the physician stated that they possibly suffered a seizure which caused the fall. This Report was requested to be filed by MSHA inspector and is under protest by this contractor.

Tire technician was in the process of dismounting a 33.00X51 tire on the LF Steer of an end dump rock truck. When technician released the lock ring, it fell and rolled around and struck the technician's right big toe.

A grass fire was started by the hot exhaust on a contractors pick-up. MSHA, fire department and shift supervisor were notified. A fire lane was plowed and the fire was extinguished.

A mechanic was installing a pull block on the rear of a scraper. Employee got finger pinched between the pull block and a backup alarm mounting plate. The employee fractured the tip of right index finger.

Lightning storm passing through the area struck hay bales in mine reclamation area. MSHA, fire department, and shift supervisor were notified. Fire department plowed fire lanes and extinguished the fire.

Operator placed lunch box in cab. As ee stepped back to exit the cab foot slipped off the step. The operator reacted by swinging arm to catch self and the back of right hand contacted the handrail. An x-ray showed a cracked bone in right hand.

Employee was rolling up power cable. EE got off the truck to tie the rope to the cable. While rolling up the rest of the cable/rope the employee was unaware ee was straddling a loop in the rope. As ee rolled the cable up leg got pinched between the rope and equipment, injuring employees left leg. Employee was unable to return to work on next scheduled shift.

While assembling a retainer on 104 dragline on left propel case. Employee was using a pry bar to adjust alignment of retainer. The bar was too long to get in a comfortable position and was being used overhead. The bar slipped smashing right thumb between bar and bull gear cover.

Worker was climbing in the back of EE's truck to get a bottle of water during break. The truck was pulling a trailer, and when the worker stepped down from truck EE's foot got caught on trailer causing worker to fall back, landing on EE's hand/wrist.

Employee was using an impact wrench with a socket secured to the impact with a welding rod. While operating the impact the employee's arm was too close to the welding rod cutting employee's forearm.

Employee was using a 36" pipe wrench to open a valve. The valve opened suddenly, employee pinched finger between the wrench and circulating pump fracturing finger.

During a wire rope resocket the employee got his left hand pinched between a pry bar and the wire rope. Stitches were required on the little finger of the left hand.

Welder was cutting a piece of plywood. Setting it down on the handrails of a walkway to draw it out, he stepped around the handrails and stepped off the walkway. His leg hit the grating on the other walkway cutting his leg that required stitches to close it up.

Pulling a transmission out from a Kress truck. The drive line guards fell off the module and landed on my left upper knee. Pushed it over my ankle to the floor.

Employee was unloading cam frame bolts (approx. 45 lbs. each) from a plastic box supplied by the mine. He stepped into the box to reach the bolts and noticed a bite/sting on his leg. He keep working. Later the bite site became painful, swollen and abscessed. He reported this to his boss 9/3/14 and was instructed to go the occupational medial clinic 9/4/14.

Lighting storm passing thru the area must have struck the separator tank on a gas well location. MSHA, fire department and the production foreman of the well were notified. The production foreman from Memorial Resource Development and the fire department made the decision to allow the fire to burn itself out.

At approximately 2:00 am, a Kress coal hauler was observed off the road in an MSHA pond. Upon investigating the truck was found to be stuck in the mud and the operator was unresponsive. The operator was later pronounced deceased by the Justice of the Peace.

The employee tripped on uneven ground causing him to hit his head on the step of a bucket truck. The employee received one stitch by a physician.

Employee was working on a 637E Scraper. He was moving the center cutting edge into position. He lost his balance and fell sideways to floor. The cutting edge fell on his right inner knee

The employee had stopped his truck to use the porta-john. As the employee was climbing down the steps his foot slipped off and he turned sideways and struck the top of his left forearm on the truck platform. He did not use the top step but went straight to the bottom step. He was placed on restrictive duty with a bruise to the top of his left forearm.

Employee dismounting equipment, his foot slipped off muddy step and struck left Knee on track. First aid given at job trailer. Knee x-ray at occupational clinic morning of 1/25/12,bone fragment seen at knee cap. Employee put on crutches with brace and released with restrictions until seen by orthopedic doctor.

Employee using hands to place Rip Rap (12" to 24" stone). When employee removed a stone another stone rolled and mashed the Left Pinky Finger against another stone causing injury. The employee was using all PPE. First Aid administered and employee taken to Occupational Clinic for treatment. Three stitches or sutures.

Working on the camshaft of a C27. While removing the bolts the socket wrench slipped and caused left hand to smash up against the camshaft. The drive gear cut left finger at base.

Individual felt irritation in his left eye while in the shop. Flushed the left eye out a few times, but still felt irritation. Took individual into the doctor for observation. Doctor removed material and prescribed anti-biotic prescription.

Employee was sitting on a trailer and felt something sting him, but did not see what it was. The bite got infected and had to go to hospital.

Heavy storms were passing through the area bringing rain, winds, and severe lightning. During this time, it was noticed that smoke was coming from the east. Upon investigation, we found that a fire had started and ignited some dry brush and pine needles. This was located in a remote, inactive part of the mine (S-area).

Tightening the C-frame bolts on the left side of the drag line. The impact torque caught and pinched the right hand between the handrail and the impact cutting a finger that required stitches.

Traveling out of the pit, antifreeze was seen coming from the radiator. The operator placed his hand on the radiator cap to tighten it, in doing so it came off getting hot anti-freeze on his arm and leg.

Employee was walking to supervisors truck at the asphalt area when he tripped over dozer tracks and hyperexteded his left knee causing swelling in the knee area. Employee was taken to the hospital emergency room where they drained his knee, gave him a pain injection, placed him on crutches and took him of work.

EE was working on a machine placing a seat into the cab when he felt pain in his lower back

EE stuck his truck in the fill area. In helping the dozer operator hook the tow cable he twisted his left knee.

Was parking truck in the staffing area at the end of the night shift. He backed into another truck already parked. He said that he was blinded by the lights of a pickup leaving that parking lot. He made no mention of personal injury and declined an examination when he was taken for the maditory drug test. Equipment damages has been estimated at between %50,000 and $75,000

When adjusting an irrigation pipe, a tye wrap needed to be cut to move the piping. A pocket knife was used, in the process the knife slipped cutting the person's left thumb requiring stitches.

Employee had finished grinding when he felt something in his left eye. Eye was washed out and nothing was found. The next morning the employee complained of his eye and something was found and had to be removed.

A miner sprained his ankle while setting a pump. This required professional medical treatment that required means of immobilizing an injured part of the body. The employee did not miss any scheduled work days.

At approximately 2:00 pm smoke was noticed coming from the north end of the Q area. The onshift supervisor investigated and called the local fire department, MSHA was notified around 2:20 pm. A call later from the onshift supervisor verified that it was not on mine property and had been caused by a tree falling over striking a power line causing a grass fire.

Three employees were positioning a 1"x8"x8' piece of plate when one end slipped off one stand and pinched the right thumb between the plate and the skid they were working on. Right thumb was fractured.

On June 23, 2010 at approximately 7:15 pm three hay bales located in the M-5 area were noticed to be burning. The bales were located on the top of a hill; it is believed that a lightning strike from an earlier thunder storm that had passed through the area was the cause. No crews were working in the area when the fire was noticed; MSHA was notified because of the unplanned fire.

Employee was setting up to replace removable floor in transformer room in Dragline. Employee fell through the floor opening when trying to reach the shackle that was located above the floor opening breaking his wrist.

EE's of Bronco Drilling company failed to return to their rig site when returning from town. This was a frequently traveled road by all Bronco EE's to the rig. Investigation revealed that all 3 EE's were on their cell phones at the time and seat belts were not being worn. EMT attendants noted impairment of all patients.

Employee was preparing to gauge a screw jack from bottom of Revolving frame and bumped a stand and the stand fell, pinching left ring finger, causing a laceration. Five stitches were administered.

Employee had something in his eye and had no idea when he got it in there or how he got it in his eye.

Employee was moving a peice of steel plate and the employee put plate down to get a better grip and the plate slipped and landed on the metatarsal part of his boot and fractured left foot.

Employee was grinding overhead and had some trash get into left eye.

Employee was removing a wire that was caught in a dozer track when wire was pulled on it cut finger.

Employee was stepping out of truck onto unlevel ground when he arrived and twisted ankle.

Mounted bulldozer to change fuel filters. The bulldozer tracks were muddy and the oiler slipped and fell off dozer track to the ground.

EE was working on equipment using a hammer to remove pin and piece of metal came off pin and embeded in left forearm.

EE received a personal emergency phone call & needed to leave work. Leaving the job site, ee was traveling on an oil & gas lease road running accross mine property. He lost control of his truck & rolled it. He got out of the truck & called his supervisor. Whe the supervisor arrived at the scene, ee was sitting on the lease road. Took him to the hospital to be examined.

A water pumper noticed smoke at the begining of his shift as he was going out in the mine. Upon investigation he found a fire on mine property in the old C-area. This area was not accessible with our water trucks, we called Harrison County and Hallsville Fire department responded. Approximately six acres of pasture burned, fire was started by assumed lightning strike.

Employee was working and started experiencing cramps in legs and cramps moved to upper body. Employee suffered from heat stress.

While dismounting bulldozer operators foot slipped on track causing him to lose contact with hand grab iron and fall hitting his left elbow, lower back and back of his left leg on dozer track.

Received a call of a fire in our South Hallsville Mine area. Fire Department responded along with the Harrison County Forestery Department, fire burned approximately 100 acres of pasture and pine trees. Fire was started by an assumed lightning storm that had passed.

Employee backing up with Dozer on the top of a pile of dirt. As the dozer made the transition over the top of the pile and began its descent, employee was turned in his seat looking back to the direction he was going and the Dozer hit the ground causing a pinching sensation in his lower back.

Received a call that one of the oil well locations had been struck by lighting, resulting in a fire; this was in an inactive part of the mine. MSHA number called, talked with MSHA EE and shortly after that another MSHA. # 1-51906711.

Operator arrived for work on 12/09/2007 and reported that he had soreness and pain in his shoulder blade area. Operator had been ripping coal in the rib end of the pit and pushing it to the easi-miner to load. He states that he took a good jolt when backing off the coal to make another push. Operator worked all of this day and part of the next before leaving work to see a doctor.

Employee lost control of truck hauling hay. Employee stepped on his brakes to slown down, this caused the truck to pull immediately to the right. Employee tried to steer the truck back on the road, but the erosion grooves on the side of the haul road were too severe, causing the truck to loose control and roll over. Employee had multiple abrasions and stitches in left hand.

Fire started while truck (coal train) was fully loaded going to the hopper. Truck was traveling up hill at approx. 6 mph. Operator called in a code, stopped truck and shut it down; fire suppression set off. Operator left cab. Truck is estimated to be a total loss at this time.

Employee was cutting discharge hose with a utility knife. Discharge hose was propped up on his leg. During process of cutting hose, the knife slipped out of the hose and cut his leg.

Mechanic was putting new bushings on a dozer cylinder. When the bushing became stuck he hit it with a hammer, which flaked off striking him in the arm. In removing the sliver of metal, the doctor required a stitch to close the entry point. Mechanic was wearing leather gloves and a face shield at time of incident.

Employee died of natural causes - myocardial infarction (Heart Attack). See MSHA's correspondence: CMH&H Memo No. HQ-06-112-A (Sec 103), Memorandum for Allyn C. Davis. Classified Degree 9.

While Operator was mowing a dry hay field, a spark apparently was created between the hay mower and a rock in the hay field. Due to the very dry conditions, and the gusty Southeasterly winds, this caused the hay field to catch on fire. No equipment or personell was damaged or injured from the fire.

Mechanic was swapping pickup beds from one truck to another and pinched his fingers between the truck frame and the box frame of the bed. This resulted in him receiving stitches in his 3rd finger and cutting the tip on his 4th finger.

Mechanic was hauling a rock guard, when stopped at the shop he noticed that the guard had shifted and was partially hanging off the back of the truck. He lifted the guard to push it back on to the bed, in doing so he caught his middle finger on his left hand pinching it between the bed and the guard. This required two stitches to repair the cut which ran under his finger nail.

Dragline oiler was removing the pump tray on 101 dragline to repair a leak. The tray was attached to the overhead crane when it was cut loose from the mounting brackets. The tray / motor shifted when it was free causing the oiler's arm to hit the unistrut that was supporting the electrical conduit cutting his R/forearm that required stitches.

Cleaning coal in ramp S2 area, dozer operator backed into a newly dug sump. Operator was wearing his seatbelt and suffered R/shoulder pain due to backing into the 12' sump at night.

EMPLOYEE WAS CHECKING A HEADLIGHT THAT DIDN'T WORK. HE HAD REMOVED THE HEADLIGHT AND WAS LOLDING IT IN HIS RIGHT HAND AND SHOOK IT TO SEE IF IT WOULD RATTLE. WHEN HE DID THIS, IT BUSTED IN HIS RIGHT HAND CUTTING HIS THUMB

Employee was doing a turn and undercut operation on hoist motor #2; at the time of the incident he was stoning the commutator. The stone kicked back and the middle finger of his right hand was caught between the stone and brush arm. Injury- fracture to tip of finger, cut on finger that required 9 stitches.

Employee was grinding metal on the dragline bucket when a small piece of metal flew under both safety glasses and face shield, lodging in right eye. Employee had the metal removed and was issued a prescription for antibotic eye drops.

A mechanic was filling a coal hauler trailer strut with grease when he lowered his dark glasses to better his vision. At that moment, the grease came out of the relief valve slightly spraying into his eyes. The local Physician prescribed the eye drops Tobramycin only as a preventative measurement against any infections.

While working on a Dart CH-160 Trailer, ee was lowering the axle on jacks and extentions and the axle began to roll off of the jacks and landed on ee's left foot and crushed it.

EE WAS LIFTING A 55 GALLON BARREL OVER A STEP WITH AN OVERHEAD CRANE WHEN THE LIFTING TONS RELEASED THE BARREL. THE BARREL SAT DOWN ON THE EE'S RIGHT FOOT RESULTING IN 2 BROKEN BONES ON TOP OF THE FOOT.

EMPLOYEE WAS USING A 3/8" HAND DRILL WHEN THE BIT BECAME STUCK. THE DRILL ROTATED OUT OF HIS HAND AND STRUCK THE BACK OF HIS RIGHT HAND RESULTING IN A FRACTURE TO THE 4TH METACARPUS.

EE DISCONNECTED A HYDRAULIC HOSE WHILE UNDER PRESSURE STRIKING HIS RIGHT FOREARM, CAUSING 2 1/2" LACERATIONS.

EE WAS REMOVING A BUSHING FROM A TRACKHOE BUCKET WHEN STRUCK IN LEFT ARM BY A FRAGMENT OF METAL. THE PIECE OF METAL WAS REMOVED AND STITCHES APPLIED TO CLOSE THE IMPACT AREA.

EE WAS REMOVING A CUTTER HEAD TIP ONT HE EASI-MINER WITH AN AIR HAMMER. THE LEFT THUMB WAS CAUGHT BETWEEN THE AIR HAMMER AND HEADER RESULTING IN A LACERATION. STITCHES WERE REQUIRED TO CLOSE T HE LACERATION./

EE WAS REPLACING WINCH LINE ON DOZER. AFTER INSTALLING THE LINE AND TYING THE KNOT IN THE END OF THE LINE, EE BEGAN TO TIGHTEN THE KNOT. THE TAIL STARTED TO SLIP THROUGH THE KNOT. WHILE ATTEMP TING TO RETIE THE KNOT, THE TAIL SUDDENLY CAME LOOSE AND STRUCK HIM IN THE MOUTH.

EMPLOYEE WAS REMOVING OIL FILTER. FILTER WRENCH WOULD NOT LOOSEN FILTER, SO HE DROVE A SCREWDRIVER THROUGH THE FILTER, BREAKING THE FILTER. WHILE REMOVING THE FILTER IT SLIPPED AND CUT HIS HAN D.

EE WAS STRUCK IN KNEE BY FLYING METAL FROM A DOZER RIPPR TOOTH DURING REPLACEMENT OF TOOTH. 2 STITCHES WERE APPLIED TO THE KNEE

EE WAS DIAGNOSED WITH CARPAL TUNNEL SYNDROME & BELIEVES THE ILLNESS TO BE WORK RELATED.

A 6" BORING BAR WAS BEING ALIGNED TO BORES WHEN A TEMPORARY TACK ON BEARING FIXTURE BROKE ALLOWING A BEARING PLATE FIXTURE TO PINCH EE LEFT HAND AND FINGERS.

A 6" BORING BAR WAS BEING ALIGNED TO BORES WHEN A TEMPORARY TACK ON BEARING FIXTURE BROKE ALLOWING BORING BAR TO FALL ON EE SHOULDER.

INJURED WAS RIGGING OXYGEN CYLINDER TO MOVE WITH MOBILE EQUIPMENT. HIS LEFT ARM WAS CAUGHT BETWEEN LIFTING EQUIPMENT AND CYLINDER WHICH CREATED A PINCH POINT AND CAUSED INJURY.

EMPLOYEE WAS ROLLING CRIBBING OFF FORKS OF VOLVOLIFT. CRIBBING ROLLED OFFF FORK AND CAME CLOSE TO EMPLOYEES FOOT AND LEG. EMPLOYEE TURNED HIS BODY QUIKLY TO AVOID THE CRIBBING AND TWISTED HIS LEFT KNEE.

EE WAS SCRAPING PAINT ON STAIRWELL WALKING BACKWARDS WHEN HE SLIPPED AND FELL.

BUMPED KNEE ON ACCESS OPENING WHILE MOVING FROM ONE COMPARTMENT TO ANOTHER INSIDE TUB. THE COMPARTMENTS ARE APPROX 3.5' HIGH AND THE ACCESS OPENINGS ARE APPROX 22" IN DIAMETER. BRUISE ON RIGHT FEMUR JUST ABOVE KNEE CAUSING SWELLING BEHIND THE KNEE CAP.

EE WAS REMOVING AN OLD TOOTH FROM THE EASI MINER CUTTING HEAD WITH A HAMMER. A PIECE OF THE TOOTH BROKE OFF AND STRUCK THE EE IN THE RIGHT THIGH. A PHYSICIAN REMOVED THE SLIVER OF METAL AND US ED STITCHES TO CLOSE THE WOUND.

EE WAS MOVING BETWEEN THE TANDEM TIRES OF A 16G MOTORGRADER WHEN HE STEPPED ON HIS LEFT FOOT WITH HIS RIGHT FOOT. HE FELL AND CONTACTED THE SHOPFLOOR WITH HIS LEFT KNEE.

EMPLOYEE WAS SWEEPING THE SHOP FLLOR AROUND A DOZER BLADE THAT HAD BEEN REMOVED FROM THE DOZER. HE STEPPED OVER THE PUSH ARM OF THE DOZER BLADE AND TWISTED HIS KNEE. HE STEPPED ON DIRT CLODS I N THE FLOOR CAUSING THE INJURY.

EMPLOYEES WERE REMOVING THE RETAINER PLATE FROM THE SWING CARRIER BEARING; THE BEARING FELL FROMTHE RETAINER, STRIKING EMPLOYEES KNEE & JAMMING HIS LEG AND ANKLE.

EMPLOYEE STATED HE EXPERIENCED PAIN AND NUMBNESSIN HIS WRIST FRO APPROX ONE YEAR. HE RELATES THIS TO THE TYPE OF WORK PERFORMED BY A MECHANIC. UPON MEDICAL EVALUATION, SYMPTOMS WERE CONSISTEN T WITH CARPAL TUNNEL SYNDROME. IS AWAITING FURTHER TREATMENT TO SEE TREATMENT IS RECOMMENDED.

EE WAS MAKING REPAIRS TO A TRUCK BED. AREA OF REPAIR HAD BRAKE LINES CLOSE. WHEN EE STARTED USING A NEW GAUGING ROD, THE BACK OF HIS GAUGING ROD ARCED ON THE BRAKE LINE CAUSING A SMALL HOLE. B RAKE FLUID SPRAYED OUT, IGNITED BY THE GAUGING ROD AND BURNING THE EMPLOYEE.

WHILE HOLDING THE BACK UP FOR THE IMPACT REMOVAL OF BOLTS, HIS BACK BECAME STRAINED CAUSING MUSCLE SPASMS.

EE WAS PUSHING A RAMP DOWN THE SPOIL SIDE OF THE PIT. THE DOZER SLID TO THE RIGHT, MADE ONE COMPLETE ROLL AND LANDED ON THE TRACKS. EE EXPERIENCED A FRACTURE TO THE L2-4 TRANSVERSE PROCESS.

EE PUSHED THE BUTTON TO LOWER AN OVERHEAD DOOR AND THE MOTOR BROKE FROM THE FRAME. THE MOTOR FELL AND STRUCK HIM ON THE RIGHT ARM. THE MEDICAL TREATMENT REQUIRED TWO STITCHES TO CLOSE THE INJU RY.

EE WAS CUTTING MATTING USING A MACHETE, WHEN THE WIND CAUGHT THE MATERIAL AND PULLED HIS HAND INTO THE PATCH OF THE MACHETE, CUTTING HIS LEFT INDEX FINGER REQUIRING STITCHES.

EE WAS DISMOUNTING THE DRAGLINE WHEN HIS HEEL ON HIS LEFT FOOT WENT INTO HOLE ON THE BENCH CAUSING A TORN ACHILLES TENDON

EE WAS FILLING MACHINE WITH LUBRICANT BY PUMPING FROM A DELIVERY TRUCK. COMING DOWN THE LADDER HE FELL OFF OF THE LADDER. HE SLIPPED WHILE COMING DOWN THE LADDER. IT WAS GETTING DARK WHEN THE ACCIDENT HAPPENED.

EE STATED THAT SHE HAD NOTICED SYMPTOMS OF REPETITVE MOTION TRAUMA FOR A PERIOD OF APPROX 2 1/2 YEARS & NOW FEELS THIS TO BE WORK RELATED. THE FIRST VISIT TO THE DOCTOR WAS ON 11/25/98, WHICH IS OUR REPORTED DATE OF INJURY.

EE WAS DISMOUNTING THE LOWBOY TRACTOR WHEN HE TWISTED HIS RIGHT KNEE. INITIALLY, NO MEDICAL TREATMENT WAS NEEDED. AS TIME PROGRESSED, EE HAD THE KNEE CHECKED BY A DR. DUE TO CONTINUED DISCOMFO RT. FOLLOWING AN MRI, TORN CARTILAGE WAS DISCOVERED AND EE HAD SURGERY TO REPRIT THIS ON 1-20-99.

EE WAS PERFORMING HIS PRE-OPERATIONAL CHECK OF THE 992 LOADER. WHEN HE WALKED AROUND THE REAR OF THE MACHINE, HIS HAND SLIPPED OFF OF THE HANDRAIL AND HE FELL ONTO THE STOCKPILE.

EMPLOYEE WAS PULLING ON A WRENCH WHEN HE FELT SOMETHING POP IN HIS RIGHT SHOULDER.

EE WAS OPERATING AN END DUMP ON THE HAUL ROAD WHEN HE HIT A SOFT SPOT IN THE ROAD CAUSING A STRAIN TO THE LOWER BACK.

EE STATED WHILE REMOVING SOME BOLTS THE PIN IN THE SOCKET BROKE & CUT BETWEEN FIRST TWO FINGERS ON HIS RIGHT HAND

EMPLOYEE WAS DRIVING #422 WATER TRUCK. AS HE PULLED ONTO THE DI RAMP, HE CONTACTED THE TOP OF THE CAB WITH HIS HEAD. THIS RESULTED INA NECK STRAIN.

EE WAS INSTALLING A WING NUT ON A FILTER ON EAST MINER #202 WHEN HIS RIGHT INDEX FINGER WAS CUT ON A PIECE OF METAL ON THE FILTER.

EE WAS INSTALLING A BEARING IN THE SOUTH ARM BUSHING OF #202 EASI-MINER WITH A 21LB SLEDGE HAMMER. A SPLITNER OF METAL HIT LEFT THUMB REQUIRING STITCHES.

EE WAS UNLOADING A 10-1/2' X 2" WIRE SLING. HE CUT THE BANDING THAT HOLDS THE SLING IN A LOOP. THE SLING RECOILED & STRUCK HIS LEFT THUMB CAUSING A LACERATION.

EMP WAS STANDING ON THE ACCESS PLATFORM ON THE EASI MINER AND FELL OFF THE MACHINE TO THE FLOOR,TWO RIBS WERE CRACKED.

EE WAS PULLING A DOZER TAIL CHAIN THROUGH A CLEVIS & USED A TWISTING MOTION WITH HIS BACK, CAUSING DISCOMFORT.

MAINTENANCE TECH RECEIVED A LACERATION TO THE LEFT HAND WHILE SHARPENING A GASKET SCRAPER WITH AFILE.

WELDER WAS ARGOUGING WHEN A PIECE OF HOT METAL BURNED THROUGH PANTS & FELL INTO BOOT BURING FOOT.

EE WAS HAULING HAY. WHILE WALKING FROM THE TRACTOR TO THE TRUCK, HIS FOOT GOT TANGLED IN SOME BALING TWINE, CAUSING HIM TO FALL.

MECHANIC WAS REPLACING A BATTERY ON THEE HYDROMULCER AND FELT A STRAIN IN THE UPPER BACK WHILE LIFTING THE BATTERY.

MECHANIC WAS INSTALLING FRONT TIRE ON DRESER END DUMP WITH TIRE MACHINE WHEN THE TIRE WAS RELEASED IT FELL BACK & STRUCK MECHANIC CAUSING A STRAIN TO THE TORSO & UPPER ARM.

EE WAS DISMOUNTING A DOZER AND TWISTED HIS KNEE.

GRINDING IN CLOSE QUARTERS W/SOCK HOOD, OBJEDCT BOUNCED INTO EYE WHEN REMOVING HOOD.

GRINDING OVERHEAD IN A TIGHT COMPARTMENT. DID NOT REALIZE ANYTHING IN EYE UNTIL WOKE THE NEXT DAY.

EE WAS LIFTING A LID OFF OF A BOX. HE SAID HE FELT A BURNING PAIN IN HIS LEFT LOWER BACK. HE THEN WAS LIFTING A PIPE STANCHION ON THE TUB AND SAID HE FELT THE PAIN AGAINST. AT THIS TIME HE REP ORTED THE PAIN TO HIS SUPERVISOR.

DRAGLINE MECHANIC WAS CHANGING DRAG ROPES AS HE BENT OVER TO ATTACH A CABLE TO WEDGE, THE DRAG ROPE CAME OUT OF SOCKET AND STRUCK LEFT LEG CAUSING A CONTUSSION SLIGHTLY BELOW THE KNEE.

MECHANIC WAS ATTEMPTING TO REMOVE OIL FROM A P&HSHOVEL. HE WAS BENT OVER CARRYING EVAC HOSE. HE DID NOT CLEAR A BRACE THAT WAS SECURE TO THE COUNTERWEIGHT & STRUCK THE BRACE WITH HIS HARD HAT, RESULTING IN A NECK AND SHOULDER CONTUSSION.

MINE MANAGER WAS HEATING SOUP IN THE MICROWAVE. WHEN SOUP WAS HOT, HE ATTEMPTED TO HANDLE THE SOUP IN A CARDBOARD CUP. WHILE REMOVING THE CUP, HE WAS ALSO HOLDING PAPERS, WHICH STARTED TO FALL . WHILE TRYING TO CATCH PAPERS, EE CRUSHED THE CARDBOARD CUP OF HOT SOUP ON HIS LEFT WRIST. THIS RESULTED IN A SECOND DEGREE BURN.

COAL HAULER OPERATOR WAS TRAVELING DOWN HAUL ROAD 2/11/96. HE SNEEZED & IMMEDIATELY STARTED HAVING CHEST PAINS. HE CONTACTED HIS SUPERVISOR & EMS WAS NOTIFIED. HE WAS TAKEN TO ER FOR MEDICAL O BSERVATIONS, WHICH FOUND HE HAD STRAINED A CHESTMUSCLE.

MECHANIC WAS ATTEMPTING TO REMOVE A SPARK PLUG FROM AN ENGINE IN A TRUCK HE WAS PULLING ON THE WIRE & WHEN IT CAME LOOSE THE MOMENTUM PULLED HIS FINGER INTO AN EXHAUST GASKET THAT HANGS DOWN F ROM THE EXHAUST MANIFOLD

EMPLOYEE WAS TRAVELING ON J RAMP ROAD LOOKING TO THE RIGHT AT WORK TO BE DONE AND CAME UP BEHIND A MOTOR GRADER.AT THIS POINT,HE SWERVED TO MISS THE GRADER,CAUGHT THE RIPPER ON THE GRADER AND WENT OFF INTO A DITCH.

MECHANIC WAS PERFORMING REPAIRS ON A 325M DRESSER COAL HAULER. HE WAS IN THE PROCESS OF MOUNTING THE TRUCK BY THE FRONT LADDER. THE MECHANIC HAD STEPPED IN OIL & WATER MAKING HIS BOOTS SLIPPER Y. WHEN HE ATTEMPTED TO CLIMB THE LADDER HIS FOOT SLIPPED OFF THE LADDER RUNG, ALLOWING HIM TO FALL TO THE GROUND BENDING HIS LEFT KNEE CAUSING A STRAIN.

MECHANIC WAS REMOVING A TRANSMISSION BY PASS VALVE FROM A D10N DOZER.AFTER REMOVING THE VALVE SLIPPED FROM HIS HANDS AND FELL FROM HIS GRIP AND FELL.THE VALVE CUT HIS RIGHT THUMB.HE WAS TAKEN TO EMERGENCY ROOM WHERE 2 SUTURES TO REPAIR.

MECHANIC WAS INJURED WHEN HIS RIGHT FOOT WAS CAUGHT BETWEEN THE BACKHOE LOADER BUCKET AND FLOOR. THE MECHANIC HAD THE BUCKET OF THE GROUND SITTING ON THE CUTTING EDGES, WHEN THE TOP OF THE BUC KET FELL 2" AND LANDING ON HIS RIGHT FOOT, BREAKING 2 TOES.

THIS MECHANIC WAS ATTEMPTING TO LIFT A BACKHOE LOADER BUCKET OFF THE FOOT OF ANOTHER MECHANIC THAT WAS TRAPPED WHEN THE BUCKET FELL.

EE MECHANIC WAS REMOVING A A/C FALL COVER WITH RAZOR UTILITY KNIFE AT ONE POINT THE KNIFE SLIPPED & THE BLADE STRUCK HE FELT INDEX FINGER LACERATING THE END WHERE 5 STITCHES WERE REQUIRED TO REPAIR REPAIR

EMPLOYEE GOT OUT OF CHAIR. WHEN SHE STOOD UP SHE FELT HER BACK CATCH AND COULD NOT STRAIGHTEN UP. PAIN CONTINUED AND WORSENED THROUGHOUT THE EVENING.

MECHANIC WAS CHANGING TEETH ON A 1224 EASIMINER.HE WAS STRIKING THE TOOTH WITH A SLEDGE HAMMER. AT THE END OF THE SHIFT, HE FELT PAIN IN HIS LEFT SHOULDER. HE VISITED THE CHIROPRACTOR ON 9-8-94. THE CHIROPRACTOR KEPT HIM OFF WORK UNDERHIS CARE UNTIL FURTHER NOTICE, WITH TREATMENT ONGOING DAILY.

EE STATES THAT HE WAS AIR ARCING ON METAL SUPPORT IN BASE OF DRAGLINE, TRIED TO MOVE METAL SUPPORT BY TUGGING ON IT, AND FELT THAT HE HAD STRAINED HIS BACK.

MECHANIC WAS MOVING THE CRANE TROLLEY ACROSS THE SHOP, BETWEEN A SHOVEL AND A DOZER. THE TROLLEY BLOCK STRUCK A "TRUCK SPOTTER" THAT WAS INSTALLED AT THE REAR OF THE SHOVEL. THE SPOTTER FELL O FF THE SHOVEL AND STRUCK THE MECHANIC IN THE LEFT ARM.

EMPLOYEE WAS MOVING PIPE INTO BASE OF DRAGLINE FOR WELDER. THE BAR HE WAS USING TO MOVE THE PIPE SLIPPED, CAUSING A PLATE TO MOVE, SMASHING A FINGER ON EACH HAND. LEFT MIDDLE FINGER (DISTAL DI GIT) HAD 2 CM LACERATION AND FRACTURE. RIGHT RING FINGER (DISTAL DIGIT) WAS FRACTURED AS WELL.

EMPLOYEE STATES THAT HE WAS PULLING, LEADS & AIRBLOWER THROUGH MANWAY OPENINGS IN REVOLVING FRAME WHEN HE FELT HE HAD STRAINED HIS BACK.

EMPLOYEE WAS USING A 20 LB SLEDGE HAMMER TO REMOVE SHOVEL TEETH. DURING THE OPERATION HE STRAINED HIS BACK. ON 8-23-94 HE WENT TO THE CHIROPRACTOR AND REQUIRED TIME OFF RELATED TO THIS INJURY.

DRAGLINE CREW WAS IN THE PROCESS OF REPLACING BUCKET TEETH ANDWEARPLATES ON DRAGLINE 102 A 1570 BE DRAGLINE. THE OEPRATOR WAS IN THE PROCESS OF KNOCKING OFF HTE WEARPLATES WITH A 20 LB SLEDGE HAMMER. THE OPERATOR HIT THE WEARPLATE WITH THE HAMMER AND A SLIVER O FMETAL FLEW OFF THE WEAR PLATE AND STRUCK THE OILER IN THERIGHT FOREARM. THE OILER WAS TAKEN TO THE E.R. ROOM WHERE 7 S

INJURED WAS REMOVING A 120LB, 3" PIN IN THE FAIR LEADS. THE PIN WAS 5' LONG WITH A SHOULDER HEAD AT THE TOP. WHILE HE WAS ATTEMPTING TO REMOVE THE PIN FROM THE BORE, THE PIN FELL INTO THE BO RE, MASHING HIS RIGHT RING FINGER AGAINST THE SHOULDERED HEAD, REQUIRING 7 STITCHES.

EE WAS GETTING OFF OF A DOZER WITHOUT USING GRAB BARS HIS HANDS WERE FULL HE LOST HIS FOOTING JUMPED TO GROUND & LANDED STIFF-LEGGED

HE WAS ATTEMPTING TO LOAD AN EMPTY 02 BOTTLE INTO THE STORAGE RACK. HE HUGGED THE 70 LB BOTTLE AND LIFTED IT ONTO THE RACK. THE BOTTOM OF THE BOTTLE DID NOT FIT COMPLETELY ONTO THE RACK AND FE LL OFF THE RACK, ONTO THE EMPLOYEE'S LEFT TOE. THE EMPLOYEE WAS TAKEN TO THE ER ROOM WHERE HE WAS EXAMINED AND FOUND TO HAVE A BROKEN TOE.

EMPLOYEE WAS TRAVELING FROM WORK AREA TO THE OFFICE FOR A SHIFT CHANGE. HE WAS TRAVELING ON A FRESHLY WATERED RED-DIRT HAUL ROAD, WHEN HE LOST CONTROL OF THE 1992 DODGE PICK-UP TRUCK AND ROLLE D OVER, LANDING ON THE CAB IN THE DITCH.

EMPLOYEE WAS CARRYING A BUCKET OF GREASE WHEN HE SLIPPED & FELL

INJURED WAS INT HE PROCESS OF PLACIGN POLY-IPE OVER THE HIGHWALL BY HAND. DURIGN THIS OEPRATION, HE STRAINE DHIS BACK WHEN HE THREW A PIECE OF POLYPIPE OVER THE HIGHWALL.

HE WAS RE-POSITIONING 6" POLY PIPE IN THE SPOIL. WHEN THE TRACTOR THAT WAS PULLING THE PIPE TURNED HARD RIGHT THE PIPE SWUNG AROUND AND HIT HIS RIGHT LOWER LEG. THE DR STATED HE COULD RETURN TO WORK ONHIS NEXT SCHEDULED SHIFT (7DAYS LATER). HE REPORTED TO WORK ON MONDAY, TUESDAY AND TEDNESDAY, AND WORKED ALL THREE DAYS. WHEN HE RETURNE DTO WORK ON FRIDAY(HIS NORMAL SHIFT). HE H

WELDER WAS SWINGING A HAMMER AND REPORTED TWISTING HIS BACK.

EMPLOYEE WAS DISMOUNTING A 85 TON END DUMP, WHEN HE MISSED THE BOTTOM STEP. THE SUDDEN JAR OF HIM HITTING THE GROUND CAUSED HIM TO PULL A MUSCLE IN HIS BACK.

EMPLOYEE WAS OPERATING A 160 TON COAL HAULER IN THE D PIT. AFTER HE WAS LOADED, HE PROCEEDED TO EXIT THE PIT VIA THE D 1 RAMP. HE DROVE INTO A WATER DRAINAGE DITCH THAT HAD BEEN CUT THE SHIF T BEFORE, JAMMING HIS BACK. HE COMPLAINED WITH BACK HURTING THE REST OF THE SHIFT, BUT NO MEDICAL TREATMENT WAS NEEDED AT THAT TIME.

EE WAS WINCHING PUMP SUCTION LINE. WHEN REPOSITIONING HANDS ON THE WINCH CRANK, THE WINCH STOP BROKE LOOSE. THE HANDLE ROTATED BACKWARDS AT HIGH SPEED, HITTING HANDS, JERKED RIGHT ARM.

EMPLOYEE WAS LOADING HAY ON A TRUCK WITH A FRONT END LOADER. A ROLL OF HAY ROLLED OVER THE FORKS AND ONTO THE CANOPY OF THE TRACTOR, CAUSING THE CANOPY TO COME DOWN AND HIT THE EMPLOYEE.

PUTTING HAND RAILING PIPE JOINT ON FORK TRUCK ININJURED THE LEFT SIDE OF HIS BACK BY STRAINING IT.

AIR BLOWN PARTICLE ENTERED LEFT EYE WHILE PERFORMING GOUGING ACTIVITIES.

EMPLOYEE WAS LOADING HANDRAIL MATERIAL ON THE REAR OF A BOOM TRUCK. THERE WAS SOME MATERIAL HANGING OFF THE REAR OF THE BED APPROXIMATELY 4 FOOT. THE EMPLOYEE WALKED AROUND THE END OF THE TR UCK, STRIKING HIS LEFT EYEBROW. THE JARR OF HITTING HIS HEAD CAUSED A NECK WHIPLASH.

EMPLOYEE WAS WALKING BY FANTAIL OF BUCYRUS-ERIE DRAGLINE. THE HOUSE FANS BLEW UP SOME DUST. A PARTICLE OF DEBRIS BLEW INTO EMPLOYEE'S LEFT EYE SCRATCHING THE CORNEA.

OPERATOR WAS LOADING SPOIL MATERIAL IN PIT (D-3). WHEN THE BUCKET WAS FULL ON THE 992 CAT LOADER, THE OPERATOR WAS BACKING OUT OF THE CUT. WHILE BACKING OUT, A PORTION OF SPOIL FELL IN ON THE ALREADY LOADED BUCKET, CAUSING THE FRONT OF THE MACHINE TO LOWER AND THE REAR TO RAISE (APP. 6 FT.). THE JOLT IN THE MACHINE CAB STRAINED THE OPERATOR'S LOWER BACK MUSCLES.

WELDER BENT OVER TO CUT BANDING ON TEFLON WHIEL WORKING ON HAUL TRUCKS-STRAINED LOWER BACK.

EE WAS UNLOADING ROCKS AND DEBRIS FROM PARKED TRUCK, WHEN HE LOST HIS FOOTING AND FELL OUT OF THE TRUCK.

EMPLOYEE WAS CHECKING THE LUBE ON SWING RACK AT PINION REMOVAL HOLE OF BUCYRUS ERIE DRAGLINE WHEN THE STEEL COVER FELL STRIKING HIM ON AND ABOVE THE LEFT KNEE.

CO-WORKER WAS DRIVING DOWEL PIN ON PROPEL CAM BUSING OF BUCYRUS ERIE WALKING DRAGLINE WITH A SLEDGE HAMMER. THE HAMMER GLANCED OFF THE DOWELL STRIKING A 2 FOOT BAR WHICH FLEW OUT OF THE HOLE, STRIKING INJURED EMPLOYEE ON THE NOSE.

EMPLOYEE WAS GRINDING AND WIND BLEW DEBRIS BACK IN EYES. SAFETY GLASSES AND FACE SHIELD WERE IN USE. DEBRIS WASHED OUT, LEAVING ULCER ON CORNEA.

WAS ASSISTING MECHANICS INSTALL BELLY PAN ON CATERPILLAR DOZER WHEN A FOREIGN OBJECT BLEW INTO HIS RIGHT EYE. AN IMPACT WRENCH WAS IN USE AT THE TIME.
