A grass fire started on Cave Springs road and burned into the old reclaimed mine area. The cause of the fire is unknown. No mining personnel were involved with fighting the fire.

The Sabine Mining Company · Struck against stationary object

Employee was replacing an axle shaft of Komatsu HD1500-7 Water Truck. Employee found the axle bushing was stuck and used a glove hand to attempt to lift the sun gear side of the axle to free it. Upon pushing upwards on the sun gear, the employee lost grip on the oily surface and EE's forearm contacted a spline on the sun gear resulting in a 1.5" laceration requiring 5 stitches.

Two employees were at the coal barn installing a wing shroud (steel plate) on a dragline bucket. The upper piece was set in place and the crane was lowered. Both employees were going to adjust the piece of steel plate by hand when it fell. It slid down the side of the bucket, hit the ground and fell over hitting the employee breaking two Metatarsal bones in EE's right foot.

While setting up to change the hoist ropes on 102 dragline a pin that secures the shackle of the snatch block was not screwed in. This resulted in the shackle spreading apart and the snatch block popping out hitting the employee in the leg breaking the left tibia.

Employee was replacing a 2' x 4' x 3/8" piece of steel plate on a bulldozer blade. The employee stood the plate up resting the 2' edge on the floor and begin walking the plate corner to corner to the center of the blade. Once they got to the center of the blade they began laying the plate down on its opposite edge (4' side)when the injured employee felt a pull in left arm.

While on a break from ERT training, two employees were looking at a pocketknife. The injured employee made a motion to point out a function of the knife at the same time that the employee holding the knife happened to flip the blade open. The knife blade cut the injured employees right index finger and sutures were required to close the laceration.

While miner was waiting for polymer to finish transferring from trailer to stationary pre treatment tank, miner turned to the right and miner's feet slipped in the wet mud causing miner to fall and fracture right leg. The wet, muddy, red clay made very slippery conditions.

Employee stepped onto a berm to observe the cut conditions. The injured employee turned to walk down the berm and stepped into a low spot in the berm. The injured employee lost balance after stepping in the low spot and fell to the ground. The result of the fall was a dislocated right ankle and a broken right fibula bone.

While finishing drag rope change out on a 1570 dragline. The winch line dropped out of the horizontal fairlead sheave, employee was guiding the cable back into the sheave. During this process employee's left thumb was pinched between the outer and inner horizontal sheaves fracturing the thumb. The employee was wearing leather gloves at the time of the incident.

While attempting to step across the hood opening on 728 end dump, the employee stepped short and fell on EE's right side, mid-torso. The fall resulted in two broken ribs.

The employee was cleaning pond over flow drain. The pipe inlet has an expanded metal barrier to catch large debris from entering the inlet. The employee was reaching down to remove large debris from barrier and was bit on EE's right index finger by a venomous snake hiding under the debris. The worker was wearing rubber style gloves when bit.

The employee lost grip on the left grab iron before foot was on the ground resulting in a fall backward. The employee's head hit the bottom part of the ripper frame, which cut the top of head.

Employee was flipping cutting edges on a D-11 bulldozer. The employee was using a 24-inch pry bar to rotate the cutting edge. When the cutting edge unexpectedly shifted employee got right pinky finger pinched between the pry bar and the shop floor. Employee was wearing leather gloves.

EE was replacing the idler arms on a D10 dozer. EE was using a RAD torquing wrench on inside bolt. The reaction arm was pointing upward. EE placed right hand on reaction arm & left hand on the trigger of the wrench. When EE applied torque pressure to the wrench, the reaction arm rotated causing right hand to be caught/pinched between the idler & the reaction arm.

Using a pry bar to remove Becket from Dump Rope Socket. Pry bar slipped, smashing employee's thumb between pry bar and dump rope sheave. An x-ray showed a fracture of the tip of employees left thumb. The employee was wearing gloves at the time of this incident.

Employee opening the rear doors on a 53' tool trailer to gain access to the inside. Employee had release and pulled on the inner latch handle and was attempting to unlatch the outer latch when the handle came toward employee striking employee in the upper right lip. Employee received 8 sutures in upper lip. No equipment damage.

EE fell in restroom, hit their head on a cabinet and caused a bump and minor cut that stopped bleeding without a bandage. As a precautionary measure, 911 was called. Due to personal medical reasons, the physician stated that they possibly suffered a seizure which caused the fall. This Report was requested to be filed by MSHA inspector and is under protest by this contractor.

Tire technician was in the process of dismounting a 33.00X51 tire on the LF Steer of an end dump rock truck. When technician released the lock ring, it fell and rolled around and struck the technician's right big toe.

A mechanic was installing a pull block on the rear of a scraper. Employee got finger pinched between the pull block and a backup alarm mounting plate. The employee fractured the tip of right index finger.

Lightning storm passing through the area struck hay bales in mine reclamation area. MSHA, fire department, and shift supervisor were notified. Fire department plowed fire lanes and extinguished the fire.

Operator placed lunch box in cab. As ee stepped back to exit the cab foot slipped off the step. The operator reacted by swinging arm to catch self and the back of right hand contacted the handrail. An x-ray showed a cracked bone in right hand.

Employee was rolling up power cable. EE got off the truck to tie the rope to the cable. While rolling up the rest of the cable/rope the employee was unaware ee was straddling a loop in the rope. As ee rolled the cable up leg got pinched between the rope and equipment, injuring employees left leg. Employee was unable to return to work on next scheduled shift.

While assembling a retainer on 104 dragline on left propel case. Employee was using a pry bar to adjust alignment of retainer. The bar was too long to get in a comfortable position and was being used overhead. The bar slipped smashing right thumb between bar and bull gear cover.

Worker was climbing in the back of EE's truck to get a bottle of water during break. The truck was pulling a trailer, and when the worker stepped down from truck EE's foot got caught on trailer causing worker to fall back, landing on EE's hand/wrist.

During a wire rope resocket the employee got his left hand pinched between a pry bar and the wire rope. Stitches were required on the little finger of the left hand.

Welder was cutting a piece of plywood. Setting it down on the handrails of a walkway to draw it out, he stepped around the handrails and stepped off the walkway. His leg hit the grating on the other walkway cutting his leg that required stitches to close it up.

Pulling a transmission out from a Kress truck. The drive line guards fell off the module and landed on my left upper knee. Pushed it over my ankle to the floor.

Employee was unloading cam frame bolts (approx. 45 lbs. each) from a plastic box supplied by the mine. He stepped into the box to reach the bolts and noticed a bite/sting on his leg. He keep working. Later the bite site became painful, swollen and abscessed. He reported this to his boss 9/3/14 and was instructed to go the occupational medial clinic 9/4/14.

Lighting storm passing thru the area must have struck the separator tank on a gas well location. MSHA, fire department and the production foreman of the well were notified. The production foreman from Memorial Resource Development and the fire department made the decision to allow the fire to burn itself out.

At approximately 2:00 am, a Kress coal hauler was observed off the road in an MSHA pond. Upon investigating the truck was found to be stuck in the mud and the operator was unresponsive. The operator was later pronounced deceased by the Justice of the Peace.

The employee had stopped his truck to use the porta-john. As the employee was climbing down the steps his foot slipped off and he turned sideways and struck the top of his left forearm on the truck platform. He did not use the top step but went straight to the bottom step. He was placed on restrictive duty with a bruise to the top of his left forearm.

Employee dismounting equipment, his foot slipped off muddy step and struck left Knee on track. First aid given at job trailer. The bales were located on the top of a hill; it is believed that a lightning strike from an earlier thunder storm that had passed through the area was the cause. No crews were working in the area when the fire was noticed; MSHA was notified because of the unplanned fire.

Operator had been ripping coal in the rib end of the pit and pushing it to the easi-miner to load. He states that he took a good jolt when backing off the coal to make another push. Operator worked all of this day and part of the next before leaving work to see a doctor.
