PROPER SAFETY PROCEDURES WERE FOLLOWED AND THE HYDRAULIC SYSTEM WAS DEPRESSURIZED PRIOR TO REMOVING TWO HYDRAULIC HOSES. THE FIRST HOSE WAS REMOVED WITH NO PROBLEM. HOWEVER THE SECOND HOSE HAD AN UNKNOWN INTERNAL BLOCKAGE HOLDING HIGH PRESSURE IN IT. THIS WAS UNKNOWN AND UNSUSPECTED TO THE PERSON REMOVING THE HOSE. WHEN FASTENER WAS LOOSENED, HYDRAULIC OIL SPRAY IN FACE CUTTING LIP

AS EE STEPPED FROM DREDGE TO WORK BOAT HE HIT HIS SHIN OF THE LOWER LEG ON A PIPE MOUNTING CAUSING A MINOR BRUISE. SIX DAYS LATER HIS LEG TURN A DARK COLOR. HE THEN WENT TO THE DOCTOR AND FOUN D HE HAD INJURED AN ARTERY IN HIS LOWER LEG WHICH CAUSED SOME INTERNAL BLEEDING. HE TOOK 3 DAYS OFF AND THEN RETURNED TO WORK ON LIGHT DUTY FOR ANOTHER FIVE DAY PERIOD.

EE HAS USING A UTILITY KNIFE TO CUT RUBBER HOSE. THE KNIFE SLIPPED AND CUT HIS THUMB, REPAIRING WATERLINE TO THE PLANT.

INJURED WAS ASSISTING MECHANIC THAT WAS INSTALLING NEW DRIVE CHAINS ON A MANITOWAC DRAGLINE,A HYDRAULIC JACK WAS USED TO TEMPORARILY HOLD ADJUSTMENT WHILE NEW CHAIN WAS WRAPPED AROUND DRIVE SP ROCKETS.THE HYDRAULIC JACK SLIPPED OUT OF POSITION STRIKING HIM IN THE RIGHT ARM AND THEN RIGHT SIDE OF THE FACE BREAKING HIS JAW BONE.
