A SUPV & LABORER CHANGING A TIRE ON A LOADER USING A BOOM TRUCK TO LIFT THE TIRE.BOOM TRUCK WAS 14' FROM OVERHEAD WIRE.THE COME-ALONG HOLDING THE TIRE SNAPPED (A METAL HOOK BROKE),THE TIRE DRO PPED,THE BOOM TRUCK CABLE WHIPPED OUT STRIKING THE ELECTRICAL CABLE.MAN HOLDING TIRE WAS ELECTORCUTED.

THE MACHINE WAS TURNED OFF FOR REPAIR OF THE CONVEYOR BELT. THE REPAIRS WERE CONSUNMATED AND THE MACHINE TURNED BACK ON THE OPERATOR OPENED THE GUARD STUCK HIS HAND IN UNDER THE BELT. HIS HA ND WAS CAUGHT BY THE BELT. THE INJURY CONSISTED OF TWO ABRASIONS AND A BROKEN ARM.
