The No. 2 entry at spad 283 roof fall. 7' high x 15' wide x 60' long.
Fire boss run on midnight shift discovered roof fall. Fall occured between day shift ended and midnight shift started. No one working underground pm shift. Size 11' long 10' wide 61' high main intake no equipment or employee involved.
Rock fell from roof and hit on his head and back. Took to hospital. Kept over-night and released to go home 12-21-09 am.
No broken bones - Took him to Clarksburg Hospital. Put 2 stitches in and released. Left hand above wrist - no broken bones.
Midnight foreman doing fire boss check discovered roof fall. Fall occurred over weekend when no one was working or in mines. Fall was approx. 6' high x 16' wide x 20' long on No. 2 belt inby 1 block where #2 belt started. Structure & belt damaged.

Roof fall. Fell on off shift. No one underground at time of roof fall. Inby survey station 17 & No. 3 line of crosscuts. In entries #1 to #5 entry.
Roof fall, about 25 feet long, 15 feet wide & 6' high above anchorage.
Using a cutting torch to remove bolts from miner was laying on back, cutting. A hot piece of metal went inot ear and burnt a hole in the ear drum.
