Employee was removing rock from mobile bridge coal chain. The rock slipped and cut and crushed the employee's little finger on his left hand.

The roof fall occurred in the 1st south cast butt room #40 righside in second x cut from # 5 entry fall was approximately 30' long 20' wide and 4' to 5' high area was bolted with 4 & 3 bolts there was a gray slick & sided clay stone streak at top of roof fall.
