Working on a fan on hydroseeder, hadn't put fan cover back on, checking fan, slipped and fell. Hand slipped into fan, catching finger tips.

Welding on 475 S/N 20052 on the pivot shaft area. Got hot on the inside and started a fire on the inside wiring that could not be extinguished.
