Tripped while replacing Rollers on plant

Changing bearing on screw/auger style washer. Auger was supported with crane. Bearing needed wedged free from auger. When bearing came loose, auger shifted pressing on left foot between auger and side wall of wash tub.

Due to cooler weather, plant having problems with belts, pulleys and material freezing up. Conveyor belt started tracking hard to south side. EE was looking at tail pulley for material build up and slipped, twisting EE's ankle.
Slipped on bulldozer track and hit knee off track while fueling up.

Employee was starting to run a bolt into the roof and a rock fell and hit their head. Employee has a laceration on the left side of the head from the rock pushing their mask into the side of their head

Pulling shot hose out of face - working on shot crew.
Co-workers just completed drilling holes in angle iron. Employee blew away metal shavings with their breath. Shaving went in their eye. Employee did have safety glasses on.
While repairing the head drive with safety glasses on, dirt and rust got in both eyes

Assisted coworker lifting hoses 8/30. Noticed L side pain next morning. Went to ER 9/1. Diagnosed w/abdominal wall strain. No treatment. Continued to work until 9/12 when off for unrelated elective surgery. Contact foreman 9/27 that ee had surgery 9/26 for hematoma thought to be caused by strain.
Was moving the man lift basket. When it stopped, it came to rest against the handrail of the processing plant catwalk. This pinched the skin off at the tip of the finger.

While trying to cut conveyor belt, employees hand slipped and utility knife cut hand requiring three stitches
Employee was helping with a concrete driveway pour. When EE stepped in wet concrete to spread it, the concrete raised higher then the boots, causing it to be in contact with EE's skin.
While attempting to lift a heavy plate to secure on the machine, the plate slipped of the machine and employee tried to catch it. This resulted in a strain to the lower back.

The hill belt was overloaded. They were unaware that gearbox on south side did not have a backstop and disconnected gear box on north side of belt by unbolting turnbuckle from conveyor frame. Gearbox started rotating backwards and struck EE. EE had broken bones in left arm, dislocated left knee, contusions on left arm and left leg, internal bleeding, collapsed lung.

Employee was closing the door of the scaler with his right hand. He inadvertently closed the door on his left thumb. His thumb is broken in 5 places.

Employee was in the man basket hanging 480 cable on roof bolts. He started to feel sore during the day. That night the pain got worse and the next day he went to a doctor.
Employee was cleaning under the lateral 2 head drive with a shovel when a clump of mud slid down and hit him in the knee. The mud caused a bruise on his knee. The injury is a sprained or bruised knee. He was given a brace and took 3 days off.

EE was checking the sight pole for an excavator operator. Some of the stone below his feet gave way and he fell into the ditch. He continued to work the rest of the day saying he was fine. He decided to go to the doctor on 4/22/2011. EE is part-time and any time off was not related to this injury.
Two workers were putting together 6" diameter yelomine pip. EE got his foot stuck in a strap, which caused him to trip, twist and fall on the ground. The resulting injury was a broken fibula in ee right leg.

Climbing off the DBT Crusher. Cut left middle finger on metal burr sticking up off the wing
Wasn't feeling well when arriving at work. Continued to feel worse throughout the evening. An ambulance was called.
While cutting a piece of shot wire, employee cut left thumb with knife.

Horizontal stress caused a roof fall
Employee was attempting to wash out the bucket on the 980 Wheel Loader he was operating and while pulling on the 1 1/2" wash out hose, lost his footing and twisted his ankle. He was taken to the hospital for treatment. Upon inspection of the area, the ground apppears relatively flat. The weather was sunny and dry. The ground may have been wet from spraying the bucket out.
Stepped on loose ladder on left rear of the machine and fell to the ground.

Injury was barring the feeder when he missed the mud he was barring. The bar hit his right pinky finger up against the feeder. He had four stitches and tore the nail off.
