INVESTIGATION OF ACCIDENT SITE REVEALED THAT A SLAB LOCATED ON RIGHT RIB NEAR FACE SPALLED OFF, ROTATING APPROX. 180 DEGREES STRIKING HIM ON HIS HEAD AND UPPER TORSO. HE SUSTAINED SEVERE CRUSH ING INJURIES. THE APPROX. SLAB MEASUREMENTS WERE 5' L X 3' W X 14" IN THICKNESS.

The miner had previously inspected and barred down the heading before drilling. An unseen rock fell from the rib onto the bench and slid down striking the driller in the back. The driller then walked to a nearby worker and they determined that he should go to the clinic for further examination.

Miner was longhole drilling and trying to replace his steel and lost control of the drill, pinching his right hand between the leg and drill. Miner was taken to clinic and X-ray showed a minor fracture of the right wrist. Miner placed on light duty with no time lost.

While re-routing a 4" compressed air line a rock, measuring 1'x1'x8" being held in place and hidden from view by the air line, fell from 9' high & hit the miner on the right hand & foot. Cut on middle toe, 6 days later, was infected & miner was checked into hospital.
