Employee picked up a 30 lb. plate and felt some soreness in their back.

Ee was prep to clean the A/C of the control tower with the water trk but while exiting the truck ee stepped on a rock and sprained left knee. The ee sought medical attn on 7/24 and was diag with a sprain and released w/o rest. MRI results were obtained which revealed a hairline fracture of the femoral condyle which made this as a med reportable injury as of 8/7/17.

Employee was attempting to start a weed eater and strained his right arm. The Device had restriction when pulling on the starter rope.

There were stones present on the ground around Bin #8 due to spillage. While walking toward the stairs, Employee was inattentive to the condition, stepped on a stone causing a fall to ground, injuring left ankle. Employee finished the shift. The next day Employee went to a clinic and an x-ray was taken, confirming a fracture.

Employee had obtained two FM-20 samples from the production cone and one belt stream sample of CM-7 in the morning. Employee then went to the QC lab where he was in the process of splitting down a sample. Employee discarded half the sample into a 5-gallon bucket (approx. 30lbs) at waist height when he felt a sharp pain in the upper part of his back resulting in a strain.

EE WAS BEGINNING TO WORK OFF A SECURED LADDER (~12-16'). HE STATED EXTENTION LADDER SAFETY LATCHGAVE WAY AND HE FELL TO GROUND.
2 EMPLOYEES WERE POSITIONING TWO DRAINAGE PIPES TO JOIN TOGETHER. IN THE PROCESS OF ALIGNING AND JOINING, ONE EE HAD HIS FINGER CAUGHT. THE IMPACT RESULTED IN A LACERATION AND FRACTURE TO HIS RIGHT RING FINGER (LAST JOINT AT FINGERNAIL).

PUSHING SHALE OFF A 30' WIDE BENCH WHEN APPROX 12 FT OF THE BANK GAVE WAY CAUSING THE DOZER TO ROLL OVER DOWN THE BANK APPROX 30 FT.
EMPLOYEE SLIPPED ON THE SNOW COVERED GROUND AS HE LIFTED ONE LEG TO BEGIN TO ASCEND THE LADDER OF A MICHIGAN 475C LOADER AND FELL, BRUISING HIS PELVIS.

EMPLOYEE WAS RE-SETTING THE TOGGLE PLATE AT JAW CRUSHER. HE WAS USING A BOTTLE JACK TO PUSH THE PITMAN FORWARD AND WHEN HE MOVED THE PITMAN OUT HE REACHED IN TOWARDS THE TOP OF THE TOGGLE PLAT E TO CHECK THE CLEARANCE WHEN THE JACK SLIPPED OFF THE PITMAN, SMASHING HIS FINGER.
ON 10-12-92, THE EMPLOYEE INFORMED SUPERINTENDENT THAT ON 10-9-92, WHILE CARRYING A 6" SUCTION PIPE FOR THE PUMP STATION THAT WAS BEING SET UP, HE FELT A PAIN IN HIS BACK. HE WENT TO HIS CHIRO PRACTOR OVER THE WEEKEND AND WAS DIAGNOSED WITH A STRAIN TO HIS UPPER BACK AND WAS RELEASED FOR DUTY WITH NO RESTRICTIONS.
WHILE PERFORMING REPAIRS ON A CONVEYOR A TAILPULLY GUARD WHICH HAD BEEN REMOVED SLIPPED STRIKING THE EE S FINGER.
54736YEE WAS ATTEMPTING TO REPLACE A V-BELT ON THE SECONDARY CRUSHER ASSEMBLY. DRIVE MOTOR WAS RUNNING BUT THE CLUTCH WAS DISENGAGED. AS HE WAS REPOSITIONING, THE CLUTCH ENGAGED. THE EMPLOYEE' 20356T THUMB WAS CAUGHT IN THE SHEAVE CAUSING AMPUTATION OF THE THUMB TIP.
