EE WAS HAULING COAL FROM THE FACE TO THE FEEDER USING A JOY 10SC END DRIVE SHUTTLE CAR. WHILE TRAMMING TOWARD THE FEEDER, THE EE REPORTS THAT THE SHUTTLE CAR STRUCK A DEPRESSION IN THE ROADWAY CAUSING HIM TOBE THROWN INTO THE UNDERSIDE OF THE OPERATOR'S CANOPY. THE OPERATOR SUFFERED A SPRAINED NECK AS A RESULT.
EE AND COWORKER WERE UTILIZING A ROOF BOLTER TO PERFORM REHABILITATION WORKG THE THIRD SOUTHEAST SUBMAINS. EE WAS STANDING BESIDE THE MACHINE WITH HIS LEFT HAND OF TOP OF IT, WHEN A PIECE OF D RAW ROCK FELL FROM THE MINE ROOF STRIKING HIS HEND. THIS REULSTED IN A HAIRLINE FRACTURE BETWEEN THE FIRST AND SECOND KNUCKLE OF HIS LEFT LITTLE FINGER. THE EE ALOS RECEIVED SUTURES TO THE FR
THE EE WAS HOLDING UP THE MINER CABLE AND WATER LINE FOR A SHUTTLE CAR TO PASS THRU A CROSSCUT WHEN THE ACCIDENT OCCURRED. EE RIGHT HAND WAS CAUGHT BETWEEN THE SHUTTLE CAR CANOPY AND THE MINER CABLE WHEN THE SHUTTLE CAR PASSED UNDERNEATH. THIS RESULTED IN A LACERATION TO HIS RIGHT INDEX FINGER. SUTURES WERE USED TO CLOSE THE WOUND. THE EE WAS WEARING METACARPAL GLOVES. NO LOST TIME

STOPPED THE MINER AND LIFTED THE 1 1/2" WATER LINE AND THE TRAILING CABLE TO PLACE THEM ON THE CABLE HOOK ATTAC HED TO THE MACHINE. EMP REPORTED THAT HE STRUCK HIS HARD HAT AGAINST THE MINE ROOF WHEN HE STOODUP WHILE LIFTING THE CABLE AND WATER LINE. REPORTED PAIN IN HIS NECK.
UNPLANNED FALLS OF ROOF OCCURRED AT 3 LOCATIONS (STATION SPAD REFERENCES #13621, #5390, AND FALLWERE IN OLD WORKINGS AND NEAR THEA REA WHERE PILLAR MINING WAS CONDUCTED. NO INJURIES AND NO DAMAGE TO MINING EQUIPMENT OCCURRED. MSHA NOTIFIED BY PHONE WHEN FALLS WERE DISCOVERED.

EMPLOYEE WAS STANIDNG BESIDE A CONTINUOUS MINER. WHEN TEH OPERATOR TRAMMED THE CONT. MINER IN REVERSE, THE EMPLOYEE WAS PINNED BETWEEN THE TRAILING CABLE RESTRAINING CLAMP AND THE COAL RIB. H E SUFFERED CRUSHING INJURIES TO THE PELVIC AREA UPPER LEGS. THE INJURED EMPLOYEE WAS OBSERVING HT EMTHANE MONITOR READOUT IN THE DEKC OF HT EMACHINEAT THE TIME OF THE ACCIDENT.

HE CONTINUED TO WORK FOR SEVERAL MINU TES BUT WAS FORCED TO STOP BECAUSE OF THE INCREASING PAIN. HE DID NOT WORK ON HIS NEXT SCHEDULED SHIFT (MAY 9, 1994). INDIVIDUAL WAS OPERATING A 21SC SHUTTLE CAR, HAULING COAL FROM NO. 6 LEFT GOING TO THE MINER HE WAS THROWN OUT OF HIS SEAT WHEN HE HIT A HOLE IN THE ROADWAY.
INDIVIDUAL WAS CARRYING A CASE OF POWDER AND STEPPED ON A ROCK, CAUSING HIM TO FALL, INJURING HIS BACK.
INDIVIDUAL WAS LIFTING A BOX OF RESIN GLUE AND FELT SOMETHING PULL IN HIS SHOULDER.
EMPLOYEE WAS ATTEMPTING TO LIFT A HYDRALIC CYLINDER WHEN HE EXPERIENCED A SUDDEN PAIN IN HIS LOWER BACK. HE MISSED WORK ON 7-16-93 AND REMAINED OFF UNTIL HE RETURNED ON 8-2-93(15DAYS OFF). HE WORKED FROM 8-2-93 UNTIL 8-10-93 FIRST DAY OFF ON RECURRENCE. HE REMAINS OFF WORK. THE OVERSIGHT IN REPORTING THIS OCCURRENCE WAS DISCOVERED DURING AN INTERNAL RECONCILATION OF ACCIDENTS.
LIFTING ON 4"X8" HEADER TO LOAD INTO SCOOP BUCKET TO BUILD OVERCAST AND STRAINED HIS LOWER BACK.
INDIVIDUAL WAS CUTTING WIRE FROM BUNDLE OF ROOF BOLTS. HE WAS STANDING BETWEN THE WHEELS OF THE ROOF BOLTING MACHINE. THE OPERATOR MOVED THE ROOF BOLTING FORWARD. THE BACK WHEELS RAN OVER T HE INDIVIDUALS RIGHT FOOT.
INDIVIDUAL WAS HELPING LOAD 42" BELT ONTO FLAT CAR. THE INDIVIDUAL WAS ON TOP OF THE BELT HOLDING IT DOWN. THE BELT CAME TO THE END OF THE OF THE ROLL, IT BECAME FOULDED PULLING THE BELT OF TH E FLAT CAR. AS THE BELT CAME OF THE FLAT CAR, THE INDIVIDUAL FELL TO THE GROUND.
INDIVIDUAL BREATHED INTO HIS LUNGS. BELT WAS SLIPPING AT B BELT HEAD WHICH CAUSED THE BELT TO PRODUCE SMOKE.
INDIVIDUAL WAS DOWN ON HIS KNEES CUTTING A PIECE OF BELT FOR A GUARD AND WHEN HE BENT OVER TO CUT THE BELT, HE FELT A PAIN IN HIS LOWER BACK.
INDIVIDUAL WAS ATTEMPTING TO MOVE THE BELT ROLLER AROUND SO THE END OF THE SHAFT WOULD BE POSITIONED TO PLACE THE BEARING ON IT. AFTER PULLING ON IT, THE PERSON COMPLAINED OF PAIN IN HIS BACK AND LOWER STOMACH.
INDIVIDUAL WAS IN THE PROCESS OF SLIDING THE TRACK (RAIL) OVER (TOO CLOSE TO BELT LINE). THE 60-POUND RAIL FELL OFF THE JACK AND DOWN ON THE INDIVIDUAL'S LEFT ANKLE.
INDIVIDUAL WAS ASSIGNED TO GET CABLE HANGERS. HE WAS TRAVELING BETWEEN NO. 2 AND NO. 5 ENTRY WHEN HE STRUCK HIS HEAD AGAINST THE MINE ROOF INJURING HIS NECK. THE AREA HE INJURED HIS NECK WAS L OWER IN HEIGHT THAN THE ADJACENT ENTRY.
INDIVIDUAL WAS HELPING ELECTRICIAN LIFT DRILL POT FROM A 3-WHEELER OVER TO THE DRILL BOOM OF THE FLETCHER ROOF BOLTER, WHILE LIFTING THE DRILL POT HE EXPERIENCED PAIN IN HIS LOWER BACK AND ADO MEN. THE INDIVIDUAL DID NOT START MISSING WORK DUE TO THE INJURY UNTIL 3/18/93.
DINDIVIDUAL WAS HELPING CHANGE OUT A JOINT OF 3IN DISCHARGE LINE & WAS WALING BY THE BELT HEAD (G-HEAD(G BELT HEAD ) SLIPPED & FELL FORWARD BENDING HIS FOOT FORWARD
INDIVIDUAL WAS LOADING ROCK DUST INTO ROCK DUSTER, HER PICKED THE ROCK DUST BAG OFF THE GROUND AND TURNED TO PUT IT ON TOP OF THE TRACK DUSTER, HE FINISHED ROCK DUSTING AND STARTED HIS REGULAR SHIP AS SHUTTLE CAR OPERATOR. HE WORKED UNTIL 6:16 WHEN HE WAS TRANSPORTED OUTSIDE BY THE BELT EXAMINER.
PERSON BEGAN MISSING TIME ON 01-29-93.
INDIVIDUAL WAS UNLOADING BELT BOX FROM ROLL BACK TRUCK AND HIS CHAIN FOULED UNDER THE POWER BOX, HE STEPPED DOWN FROM THE FORK LIFT AND TRIED TO PULL THE CHAIN STRAINING HIS BACK.
INDIVIDUAL WAS OPERATIN A ROOF BOLTER, WHILE DRILLING THROUGH A CURTAIN BOARD AND WAS PULLING HIS DRILL STEEL DOWN THROUGH THE CURTAIN BOARD HE HAD ROTATION ON AND WAS PULLING ON THE STEEL AND HIS RUBBER GLOVE GOT HUNG ON THE STEEL AND TWISTED HIS HAND HURTING HIS LITTLE FINGER. INDIVIDUAL DID NOT START MISSING WORK UNTIL 3/4/93.
INDIVIDUAL WAS GRINDING (WITH DISC GRINDER) AWAY SLAG AND WLED FROM METAL BEING USED TO REPAIR 482 SCOOP. THE GRINDER HUNG ON THE OLD JAGGED WELD AND KICK OUT. THE GRINDER CAUGHT HIS LITTLE FINGER TEARING A HOLE IN THE SIDE OF IT.
