IE was climbing a fixed ladder on the crusher structure to reach the next landing when a self-closing safety gate above, misaligned and protruding into the ladder access area, struck EE's mouth/face as EE looked up. Impact chipped part of EE's upper right front tooth. IE descended safely and reported injury. Gate was removed from service and repaired/repositioned.

Texas Materials Group Inc · Caught in, under or between a moving and a stationary object

While offloading a poly-fusion machine from a equipment trailer, the employee lost control on the ramp while pulling it toward themselves. The machine rolled over the employee's right foot. Upon freeing their foot, the employee lost balance but recovered without falling. The employee then grabbed their right buttock, indicating pain, and reported discomfort to another employee.
