EMPLOYEE WAS CUTTING RUBBER ON THE SCREEN PLANT. THE RAZOR KNIFE SLIPPED AND STRUCK HIS LEFT THUMB RESULTING IN A LACERATION REQUIRING SEVEN SUTURES.

EE WAS TIGHTENING BOLTS ON SCREEN DECK WHEN WRENCH SLIPPED CAUSING HIM TO LOOSE BALANCE. WHILE REGAINING BALANCE HE STRUCK THE SCREEN DECK MOUNTING BRACKET WITH HIS LEFT ANKLE RESULTING IN AN ABRASION AND CONTUSION THROUGH THE LEATHER OF HIS STEE-TOED BOOT. EE WAS USING ALL REQUIRED PERSONAL PROTECTIVE EQUIPMENT AT THE TIME OF INJURY.
