pulled handle to close gate to screen plant distribution box, finger got in between handle and stop at pinch point

While working on the crusher distribution box, went to move the 1-inch plate (distribution divider). The plate broke loose and crushed left thumb fracturing it.

After cleaning the bucket on a Skid Steer. The employee went to get in the equipment to park it for the night and stuck left shin on the step on the bucket causing a laceration and the need for stitches.

EE TOOK A SHORTCUT AN CRAWLED UP A CONVEYOR BLET TO UNHOOK A COME-ALONG. AFTER UNHOOKING THE COME ALONG HE FELT HIMSELF SLIDING DOWN THE BELT. THE BELT COLLAPSED BETWEEN THE ROOLS FROM HIS WEI GHT CAUSING HIM TO FALL APPROX. 6 FEET ONTO HIS BACK.
