Tow Path Ready Mix · Caught in, under or between running or meshing objects

IE and another EE were adjusting the tracking of a conveyor belt. After completing that task, IE went inside the guard and reached into the conveyor belt to clean out material. A roller grabbed the pliers he was using and pulled his hand and arm inside. The other EE hit the E-stop, but the hand and arm was severely damaged.

The EE/miner was driving a wheel loader from the sand and gravel pit to the parking area. The windows on the loader were open and allowed dust to enter the cab. The dust came into contact with the miners eye. The miner disregarded the incident until the next day when his right eye became irritated.
