A ROCK WAS DUMPED INTO THE HOPPER WHICH WAS TOO BIG FOR THE HOPPER/CRUSHER TO HANDLE. LOADER OPERATOR CLIMBED LADDER AND WAS IN HOPPER WHEN EE FELL FROM TOP OF LADDER AT HOPPER TO THE GROUND.
