Employee was helping remove a panel from a screen when the screen panel jerked their arm down and over extending upper arm causing pain in shoulder. The incident occurred on 6/11/2025 but the employee's shoulder became worse over time and they had to receive surgery on 6/23/2025.

The mechanic was removing the driver side platform from a haul truck. The platform was secured to the shop overhead crane. When the last bolt was removed the platform rotated and the edge of the platform cut the top of the mechanic's head.

After completing work with an excavator, the employee was dismounting the parked machine. As employee stepped from the step to the ground employee felt a pain in right shoulder with which employee was holding the hand rail. The employee did not see a doctor until 02/18. Employee submitted to an MRI and results were read on 02/19/18. Employee has surgery planned for 02/22/2018.
The Pit Foreman, was discovered in the restroom unresponsive and without a pulse.

Employee cutting wire mesh with shears when shears slipped and employee cut his finger on exposed wire mesh.
When turned on, the air bubble in the washdown hose caused the hose to twist in the hand of the employee and the nozzle attachment band cut the employee's right hand at the base of his thumb.

Employee was tightening the main shaft nut when the tightening device bounced off of the knocker wrench and his right hand ring and little finger were pinched against the main shaft nut, requiring stitches to both, and a break on the tip of the little finger.

Employee was clearing a rock slab from a chute when the slab rolled over and pinched his right hand against the side of the chute, injuring his right middle and ring fingers.
While changing screen cloth injured employee's left hand was struck by a hammer being used by another employee.

Supervisor was cutting a plastic tie-down which was securing an electrical shutoff line to a pit pump hose when the knife cut through the tie-down and punctured the left forearm of the employee. 3 sutures were required to close the wound.

Employee stepped down from the ladder of the water truck and turned his ankle, resulting in a severe sprain of the right ankle.
Employee has involved in a physical altercation with another employee who pushed the injured employee against a wall and wrestled him to the ground injurying his left shoulder and right knee (muscle strain and bone bruise).
Employee was climbing into the cab of a Hitachi shovel when his left foot slipped and his left hand gave way, causing his entire weight to pull against his right shoulder.
While preparing a cutting torch to cut bolts on a gravity take-up pulley in the plant, the employee attempted to clear a clog from the tip of the torch with his gloved hand. The torch ignited while the tip was still in contact with his hand, causing 2nd degree burns to his left hand at the base of his middle finger.

THE EE SUFFERED LACERATIONS TO HIS ARMS AND THE BACK OF HIS HEAD REQUIRING STITCHES AND A FRACTURE TO HIS LOWER BACK L-6 AREA REQUIRING THE USE OF A BACK BRACE AND B
WHILE USING A PRY BAR TO REPOSITION EX1100 SHOVEL TRACKS FOR LOADING, THE TRACK SHIFTED AND PULLED THE PRY BAR FROM THE EMPLOYEE'S HAND AND STRUCK HIS LEG ABOVE THE RIGHT KNEE. THE SUDDEN MOVE MENT CAUSED A MUSCLE STRAIN TO HIS LOWER BACK.

AN ENCLOSED CAB DRILLER STEPPED OUT OF HIS CAB TO THE ADJUSTING WORK PLATFORM TO LOOSEN THE PIPE JOINT ON HIS DRILL STEEL. WHEN HE STEPPED BACK TOWARD THE CAB CONTROLS HE STEPPED IN THE OPENIN G BETWEEN THE CAB AND THE WORK PLATFORM. HE BRUISED HIS RIGHT WRIST AFTER SLIPPED DOWN.
EMPLOYEE WAS STANDING IN FRONT OF 2 FT HIGH CONCRETE BLOCKS. WHEN HE TURNED TO WLAK AWAY FROM JOB, HE TRIPPED OVER THE CONCRETE BLOCK, FALLING ON HIS RIGHT ARM. HE BROKE HIS ARM ABOVE THE ELBO W, BUT RETURNED TO WORK WITH RESTRICTED DUTY.
EMPLOYEE STRAINED HIS BACK WHILE USING A CHAINSAW AND LIFTING LOGS. THE IRRITATION OF THE BACK INJURY WAS REPORTED THE FOLLOWING DAY. THE EMPLOYEE WAS PLACED ON RESTRICTED DUTY ON 3-20-01. HE WAS NOT CERTAIN HOW OR WHEN HE HURT HIMSELF.
A ROCK FALLING FROM THE SHOVEL BUCKET STRUCK THE BED OF THE HAUL TRUCK IN SHICH THE EE WAS SITTING WHILE BEING LOADED. THE MOVEMENT OF THE TRUCK CAUSED THE EE TO STRAIN MUSCLES IN HIS NECK.

EE WAS HOLDING KNOCKER WRENCH ON NUT. SLEDGE HAMMER WAS BEING USED TO LOOSEN BOLT. HAMMER GLANCED OFF WRENCH STRIKING EE ON THE FINGER.
EMPLOYEE WAS WASHING DOWN CONCRETE PAD WHEN HE STEPPED ON THE EDGE OF THE PAD. THE PRESSURE FROM THE WATER HOSE PUSHED HIM OFF BALANCE CAUSING HIM TO TRIP BACKWARDS STRIKING HIS HEAD ON A ROC K WHEN HE FELL. THE CONCRETE PAD WAS APPROX 12" ABOVE THE GROUND. THE INJURY RQUIRED 9 STITCHES.
VENT GUARD ON 988 F LOADER WAS MISSING FROM AIR CONDITIONING UNIT. EE REACHED OVERHEAD TO ADJUST AIRFLOW AND ACCIDENTALLY STUCK HIS FINGER IN VENT CONTACTING A/C FAN BLADES. STITCHES REQUIRED FOR MEDICAL TREATMENT.

WHILE EE WAS WAITING TO SERVICE MOBILE DRILL AND DRILL WAS BEING LOWERED FROM SUPPORTS TO TRACKS, EE'S LEFT FOOT WAS CAUGHT UNDER TRACKS AS THEY CONTACTED THE GROUND. THE SECOND TOE ON THE LEF T FOOT WAS PINCHED BY THE CONTACT.
EE WAS TIGHTENING BOLTS ON JAW CRUSHER. LARGE SPUD WRENCH BEING USED SLIPPED FROM WORN BOLTS. EE S RIGHT HAND CONTACTED EXPOSED THREADS ON END OF BOLT, CUTTING HAND BETWEEN THUMB AND INDEX FIN GER.
EE WAS RELOCATING CRANE. HE HAD HIS FOOT RESTINGON THE HOIST BRAKE PEDAL. THE PEDAL PUSHED BACK VIOLENTLY BENDING HIS LEG. THIS CAUSED A FRACTURE OF FIBULA BONE IN RIGHT LEG
EE STATED HE WAS DRIVING A STEEL PIN INTO A CONCRETE FORM WITH A HAND HAMMER. THE PIN BROKE CAUSING A PIECE OF THE PIN TO PENTRATE THE RIGHT HAND BETWEEN THE MIDDLE AND INDEX FINGER.

EMP STATED THAT HE WAS USING A TORCH TO CUT A STEEL PIPE. BEFORE THE PIPE WAS COMPLETELY CUT INTO, IT BROKE. THIS CAUSED HAND TO BE MASHED BETWEEN PIPE BEING CUT AND ANOTHER PIPE NEAR BY.
EMPLOYEE WAS HELPING REPLACE A RETURN ROLLER ON A CONVEYOR. HE WAS USING A PRY BAR TO HELP INSTALL THE ROLLER WHEN THE BAR SLIPPED. THIS CAUSED HIS ELBOW TO HIT THE CONVEYOR FRAME.
EE WAS USING A PRYBAR TO HELP REPLACE A CONVEYOR ROLLER, WHEN THE BAR SLIPPED IT MASHED HIS FINGER AGAINST THE CONVEYOR FRAME RESULTING IN A FINGER LACERATION. THIS LACERATION REQUIRED STITCHE S.
EMPLOYEE WAS INSTALLING A GUARD ON THE V-BELT DRIVE OF THE ALTERNATOR OF AN AIR COMPRESSOR. THIS GUARD CAME IN CONTACT WITH THE BATTERY CABLE. THIS RESULTED IN A BURN TO THREE FINGERS OF THE R IGHT HAND.
EMPLOYEE WAS CLEANING BOLT HOLE THREADS ON THE INSIDE OF A CONE CRUSHER. HE HAD SPRAYED WD-40 SOLVENT INTO THE THREADS TO HELP LOOSEN THE RUST AND SCALE. LATER HE PICKED UP THE TORCH, SINCE IT WAS HANDY, TO BLOW THE LOOSENED MATERIAL OUT OF THE HOLE. THE OXYGEN AND WD-40 CAUSED A FLASH WHICH RESULTED IN A BURN TO THE RIGHT ARM AND FACE OF THE EMPLOYEE.
