EE WAS INSTALLING A ROOF BOLT, WHEN A SMALL PIECE OF ROCK BROKE, SLID DOWN A ROOF STRAP AND STRUCK HIM ON THE SHOULDER. THE ROCK FELL APPROX. 2. MEASURED 2" THICK AND WAS 2' LONG.

EMP WAS ATTEMPTING TO LEAVE THE OFFICE TO DELIVER A MESSAGE TO ANOTHER EMP WHO WAS OUT IN THE MINE YARD. STEPPED OUT OF DOOR. EMP WAS UNAWARE THAT STEPS WERE BEING REBUILT CAUSING EMP TO FALL APPROX 3' STRIKING LEG ON A PIECE OF ANGLE IRON.ANGLE IRON WAS BEING USED TO CONSTRUCT THE NEW STEPS, CAUSING THE INJURY.

EE WAS TRYING TO START A HOLE IN THE MINE ROOF TO INSTALL A ROOF BOLT. WHEN HE STARTED THE ROTATION ON THE DRILL POT, A PIECE OF ROCK DISLODGEDFROM THE TOP AND GLANCED OFF THE DRILL STEEL STR IKING HIS HAND CAUSING THE INJURY.
