WHILE WELDING A LINER TO A CHUTE IT WAS NOTICED THE LINER NEED TO BE REPOSITIONED. THEY DECIDED TO WELD A BLOCK TO BOTTOM OF HUDCO TO AID IN REPOSITIONING. UNKNOWINGLY THEY WELDED THE BLOCK TO OVERLAY. WHEN THEY RELEASED THE LINER THE BLOCK GAVE WAY AND SMASHED HIS LEFT FOOT.
