EE WAS BACKING DOWN A RAMP ON A WORKSAVER. THE WORKSAVER BECAME UNSTABLE & HE STRUCK A PARKED FORK TRUCK. HIS RIGHT LEG WAS OUTSIDE THE WORKSAVER & WAS PINCHED BETWEEN IT & THE PARKED FORK TRU CK. DUE TO THE UNSTABLENESS OF THE WORKSAVER, HE LOST HIS BALANCE, CAUSING HIS RIGHT LEG TO BE OUT OF THE WORKSAVER DURING THE COLLISION.

THE COMPANY DR X-RAYED THE LOWER BACK AND PRESCRIBED NALFON FOR INFLAMMATION AND RESTRICTED LIFTING OVER 25LBS FOR 4 DAYS.
