THE VICTIM WAS ATTEMPTING TO REPAIR THE HOIST MECHANISM ON THE BED OF A DUAL-AXLED DUMP TRUCK. THEY HAD WHICH CONTAINED APPROX. 7 TONS OF WASTE ROCK, WAS BEING HELD IN A RAISED POSITION BY A W OODEN POST AND A FORK LIFT, BOTH LOCATED ON THE DRIVERS SIDE OF THE TRUCK BETWEEN THE BED AND THE FRAME. THE HOIST CYLINDER MECHANISM FAILED WHERE ATTACHED TO THE TRUCK FRAME, ALLOWING THE BE
