Employee was bolting top in the # 5 entry, a low spot, employee stated that he slipped and his right hand went in where the scissors were on the pot. Mashed hand and pinky finger. Metacarpal Gloves were worn.

Roof fell between spad # 853 and 854. Fall was about 25' X 30' X 7' thick. Fell up to the coal rider. In the #4 Belt Entry.

AN UNINTENTIONAL ROOF FALL HAS OCCURED ON MAIN SECT-001. DUE TO INTERRUPTION OF ELECTRICAL SERVICE FROM 1-4-94 - 1-8-94 WE COULD NOT ENTER MINE. FALL AREA HAD COAL RIDER SEAM APPROX 10' ABO VE ACTIVE SEAM, WITH WATER COMING IN ALSO. TH EBOVE MENTIONED CONDITIONS AND NOT BEING ABLE TO CHECK MINE ON DAILY BASIS CONTRIBUTED TO ROOF FALL.
