Employees arm was caught when the shuttle car slid into the rib.

Employee stated he was walking from #3 to #4 to get a piece of curtain. Struck his back on a roof bolt and plate hanging down about 6" from mine roof. Employee had scratches and abrasion along with neck strain.

Employee stated he was tramming s/c to c., that was loading in #1 Entry. Employee stated the Rub Rail at open six Tram Motor swung out & went into Rib on corner of #2 entry causing employee to be thrown out of s/c. Employee's wrist went under S/C deck.

Appalachian Premium Fuels LLC · Struck by falling object

Employee was installing a bolt in the metal strap. Employee moved to side of canopy to re-position bolt machine. The foreman pulled front tilt lever instead of tram lever. The canopy dropped striking employee on his head.
