WHILE INSPECTING THE SPIRALS OF THE WASHER MACHINE THE SPIRAL WAS JAMMED WITH SAND, HE PULLED THE WATER HOSE TO FLUSH IT WHEN HIS FEET BECAME TRAPPED IN THE WATER HOSE. HE LOST HIS BALANCE AND WHILE FALLING WAS ABLE TO GRAB A SIDE LADDER. HIS LEFT FOOT GOT CAUGHT BETWEEN THE LADDER AND THE SPIRAL AND SPRAINED HIS LEFT KNEE,LEG AND INJURED HIS LOWER BACK AND HE HIT HIS LEFT ARM. HE

THE LOADER BRAKES HAD BEEN FIXED. A LEAKAGE OF BRAKE FLUID CONTINUED AND THE MECHANIC WAS CALLED TO FIX IT. THE OPERATORS WERE DOING THE INSPECTIONS AT THE BEGINNING OF EACH SHIFT AND ADDING T HE NECESSARY BRAKE FLUID. THIS OPERATOR DID NOT MAKE THE BEGINNING INSPECTION AND THE EQUIPMENT RAN OUT OF BRAKE FLUID CAUSING THE BRAKE FAILURE.

THE SAND WAS ADHERED TO THE SILO'S WALLS. THE EE, WHO HAD A PREVIOUS BACK CONDITION, WAS HAMMERING THE WALLS OF THE SILO TO LOOSEN THE SAND. THE HAMMER WAS OF APPROSIMATELY 10 POUNDS.

THE EMPLOYEE WAS CLEANING THE CONVEYOR'S BELT, REMOVING THE EXCESS OF SAND IN THE DRYER MACHINE. THE GLOVE OF THE RIGHT HAND WAS TRAPPED BETWEEN THE BELT AND THE ROLLERS. WHEN HE PULLED BACK H IS HAND, HE INJURED HIS RIGHT ARM WITH THE FRAME OF THE CONVEYOR, CAUSING A THREE STITCHES LACERATION AND A SMALL ABRASION.

THE EMPLOYEE WAS WORKING IN THE SCREW CONVEYOR. HE WAS INSTALLING THE SCREW TO THE GEER REDUCTOR USING A 36 INCH PIPE WRENCH. THE PIPE WRENCH SLIPPED DOWN AND HIT HIS FINGER.
THE EMPLOYEE WAS GRINDING A STEEL PLATE WITH A HAND GRINDER AND A STEEL PARTICLE GOT INTO HIS EYE (LEFT). HE WAS USING SAFETY GLASSES INSTEAD OF THE SAFETY GOGGLES PROVIDED FOR THAT TYPE OF JO B. IT RESULTED IN 16 MANHOURS LOST TIME.
